What Is Quetiapine?
Quetiapine fumarate is a second-generation (atypical) antipsychotic available in immediate-release (IR) tablets (25 mg, 50 mg, 100 mg, 150 mg, 200 mg, 300 mg) and extended-release (XL) tablets (50 mg, 150 mg, 200 mg, 300 mg, 400 mg). The XL formulation enables once-daily dosing and is associated with a more gradual peak-to-trough plasma level variation, which can improve tolerability and adherence compared to the IR formulation.
Quetiapine is one of the most widely prescribed antipsychotics globally, used across schizophrenia, bipolar disorder, and — more contentiously — as an adjunct in treatment-resistant depression. Its broad receptor activity profile produces antipsychotic, mood-stabilising, anxiolytic, and sedative effects. Notably, quetiapine has a relatively low propensity for extrapyramidal side effects and hyperprolactinaemia compared to many other antipsychotics, making it better tolerated in some patient groups.
Quetiapine is a prescription-only medication in the UK. Initiation requires specialist psychiatric or medical assessment.
What Conditions Is Quetiapine Used For?
Quetiapine is indicated for:
treatment of acute psychotic episodes and long-term maintenance treatment to reduce relapse risk
treatment of moderate-to-severe manic episodes as monotherapy or as an adjunct to mood stabilisers
one of the few medications with specific evidence and licensing for bipolar depression, where antidepressant monotherapy is generally contraindicated
prevents recurrence of both manic and depressive episodes
licensed as an augmentation strategy in adults with MDD who have had an inadequate response to antidepressant therapy alone
licensed as adjunctive therapy in GAD; used off-label in clinical practice
In schizophrenia and bipolar disorder, quetiapine typically needs to be taken long-term. Abrupt discontinuation carries significant risks of psychiatric relapse and cholinergic rebound symptoms. All dose changes should be made under psychiatric supervision.
How Does Quetiapine Work?
Quetiapine's broad clinical utility reflects its activity at a wide range of neurotransmitter receptors:
Dopamine D2 receptor antagonism: Quetiapine blocks D2 receptors in the mesolimbic pathway, providing antipsychotic efficacy against positive symptoms. Importantly, it has a relatively low D2 receptor occupancy compared to other antipsychotics and dissociates rapidly from the D2 receptor (the "fast-off" hypothesis) — which is believed to explain its low propensity for extrapyramidal side effects and hyperprolactinaemia.
Serotonin 5-HT2A receptor antagonism: Potent 5-HT2A blockade modulates dopaminergic activity in cortical and limbic circuits, contributing to efficacy against negative symptoms, mood disturbance, and anxiety, and further reducing extrapyramidal risk.
Histamine H1 receptor antagonism: Quetiapine's high affinity for H1 receptors is responsible for its potent sedative and sleep-promoting properties — a clinically useful effect in patients with comorbid insomnia but a significant source of daytime sedation and weight gain.
Alpha-1 adrenergic antagonism: Contributes to orthostatic hypotension, particularly at initiation and following dose increases.
Active metabolite (norquetiapine): Norquetiapine, the principal active metabolite, inhibits the norepinephrine transporter (NET), potentiating noradrenergic and dopaminergic neurotransmission in the prefrontal cortex — a mechanism likely contributing to quetiapine's antidepressant activity.
Dosages & Administration

Administration Tips
IR tablets can be taken with or without food. XL tablets should be taken without food or with a light meal.
XL tablets must be swallowed whole. Do not crush, chew, or split extended-release tablets, as this destroys the modified-release mechanism and delivers the full dose immediately.
Take XL tablets at the same time each evening. Consistent evening dosing minimises daytime sedation and optimises tolerability.
Never stop quetiapine suddenly. Abrupt discontinuation causes significant cholinergic rebound (nausea, vomiting, insomnia) and risks psychiatric relapse. Always taper under supervision.
Rise slowly from sitting or lying positions. Orthostatic hypotension is common, particularly at initiation; changing position slowly reduces the risk of falls and dizziness.
Side Effects of Quetiapine
Most side effects are mild and resolve on their own. Serious side effects are rare but require immediate medical attention.

- QT interval prolongation and arrhythmias --- baseline ECG recommended in at-risk patients
- Neuroleptic malignant syndrome (NMS) --- rare but life-threatening; fever, severe rigidity, autonomic instability, altered consciousness; requires emergency care
- Tardive dyskinesia --- risk lower than with first-generation antipsychotics but present with long-term use; involuntary movements of the face, tongue, and limbs
- Cataracts --- lens changes have been observed in long-term quetiapine use; slit-lamp eye examinations are recommended every 6 months during prolonged treatment
- Hypothyroidism --- thyroid function should be monitored during long-term treatment
- Dysphagia and aspiration --- oesophageal dysmotility and aspiration have been reported; caution in at-risk patients
Quetiapine is associated with significant metabolic side effects including weight gain, dyslipidaemia, and new-onset diabetes. Active monitoring and management of metabolic parameters is mandatory throughout treatment.
Drug Interactions
Quetiapine may interact with other medications, including strong CYP3A4 inhibitors and nephrotoxic agents. Always inform your clinician about all medications you are taking.

Important Warnings
Though rare with quetiapine, NMS is a medical emergency. Unexplained fever, muscular rigidity, autonomic instability, and altered mental status in a patient on quetiapine require immediate discontinuation and emergency assessment.
Quetiapine causes clinically significant weight gain and metabolic disturbance in a significant proportion of patients. Active monitoring and management of metabolic parameters is mandatory throughout treatment.
Lens changes have been documented with quetiapine. Baseline and 6-monthly slit-lamp eye examinations are recommended during long-term treatment, in line with prescribing guidelines.
As with all antipsychotics, quetiapine is associated with increased risk of cerebrovascular events and mortality in elderly patients with dementia. Use in this population must be short-term, at the lowest effective dose, with documented informed consent.

Quetiapine's receptor occupancy profile is dose-dependent. At lower doses (25–100 mg), H1 antihistamine effects dominate, producing sedation. At moderate doses (150–300 mg), antidepressant and anxiolytic activity increases via norquetiapine. At higher doses (400–750 mg), dopamine D2 blockade becomes clinically significant, producing full antipsychotic effect. This explains why doses for depression augmentation or insomnia are much lower than those used in schizophrenia.
Sedation is largely due to quetiapine's potent histamine H1 receptor blockade. It is most pronounced at lower doses and typically improves after the first 1–2 weeks as tolerance to this effect develops. Taking the dose in the evening helps manage daytime sedation. If sedation is severe or persistent, speak to your clinician.
Weight gain is a recognised and common side effect of quetiapine, related to H1 blockade and metabolic effects. The degree varies between individuals. Regular monitoring of weight and metabolic parameters, alongside dietary and lifestyle support, is an integral part of quetiapine management.
Alcohol significantly amplifies quetiapine's sedative effects and increases the risk of falls, respiratory depression, and other adverse effects. It is strongly advisable to avoid alcohol during quetiapine treatment.
This depends on your diagnosis. In schizophrenia and bipolar disorder, long-term maintenance therapy is typically required to prevent relapse. In MDD augmentation or GAD, duration is reviewed regularly. Your psychiatrist will guide decisions about duration based on your individual history and response.
