It is one of the most widely prescribed antipsychotics globally, licensed across a broad range of serious psychiatric conditions.

Developed by AstraZeneca, quetiapine acts on multiple neurotransmitter systems — principally dopamine and serotonin — but also histamine, adrenergic, and muscarinic receptors, producing its broad spectrum of clinical effects. How Does Quetiapine Fumarate Work?

Quetiapine exerts its therapeutic effects through antagonism of multiple receptor systems:

Dopamine D2 receptor blockade in mesolimbic pathways is the primary mechanism underlying antipsychotic efficacy — reducing excessive dopaminergic activity that drives psychotic symptoms such as hallucinations and delusions. Quetiapine's relatively low D2 affinity and fast receptor dissociation (compared to first-generation antipsychotics) is thought to account for its low propensity to cause extrapyramidal side effects and tardive dyskinesia.

Serotonin 5-HT2A receptor antagonism modulates dopamine release in the nigrostriatal and mesocortical pathways, further reducing extrapyramidal effects and contributing to improvement in mood and negative symptoms.

Administration Tips

XL tablets must be swallowed whole — do not crush, split, or chew. This destroys the extended-release mechanism.

IR tablets can be taken with or without food. XL tablets are best taken without a high-fat meal, which significantly increases absorption and could raise plasma levels unexpectedly.

Titrate slowly. Dose increases should follow the prescribed titration schedule — rapid escalation increases the risk of orthostatic hypotension, sedation, and other side effects.

Take quetiapine at the same time(s) each day. Evening dosing is common as it leverages sedative properties to support sleep.

Do not stop quetiapine abruptly. Discontinuation should be gradual and supervised — abrupt withdrawal can cause insomnia, nausea, vomiting, and rebound psychiatric symptoms.

Antipsychotic medications — including quetiapine — are associated with an increased risk of mortality and stroke when used in elderly patients with dementia-related psychosis.
