Together, these three components provide a broad, multi-mechanism approach to treating bacterial ear canal infections and associated inflammation.
Otomize is specifically formulated as a metered-dose spray, which delivers a precise, consistent amount of medication deep into the ear canal with each actuation — offering a practical advantage over conventional ear drops for some patients, particularly those who find it difficult to self-administer drops without assistance. Each spray delivers a metered dose of the combination directly to the infected canal.
The acetic acid component creates an acidic environment in the ear canal that is inherently hostile to the growth of bacteria and some fungi, complementing the direct antibacterial action of neomycin and the anti-inflammatory effect of dexamethasone.

What Conditions Is Otomize Used For?
Otomize is licensed for:
bacterial infection of the external ear canal (swimmer's ear), causing ear pain, discharge, itching, and canal swelling and oedema.
prolonged or recurrent external ear canal infection and inflammation.
inflammatory skin conditions of the ear canal complicated by bacterial superinfection.
Otomize contains neomycin — an aminoglycoside antibiotic with known ototoxic potential if it contacts the middle ear through a perforated eardrum. This preparation must not be used if you have a perforated eardrum, unless it has been specifically assessed and confirmed safe by an ENT specialist. Always inform your clinician of any history of eardrum perforation, ear surgery, or ear tubes.
How Does Otomize Work?
Dexamethasone 0.1% is a highly potent synthetic glucocorticoid. Applied topically, it binds to glucocorticoid receptors in the ear canal epithelium and subepithelial tissue, suppressing the production of inflammatory mediators including prostaglandins, leukotrienes, and pro-inflammatory cytokines. This reduces the erythema, oedema, pain, and pruritus associated with otitis externa, improving canal patency and allowing the other components to penetrate effectively.
Neomycin Sulphate is a broad-spectrum aminoglycoside antibiotic. It irreversibly binds to the 30S ribosomal subunit of susceptible bacteria, causing misreading of mRNA and inhibiting bacterial protein synthesis, resulting in bactericidal activity against a range of gram-positive and gram-negative organisms commonly responsible for otitis externa.
Glacial Acetic Acid 2% creates an acidic pH environment in the ear canal (the normal healthy ear canal has an acidic pH of approximately 4–5). This acidity directly inhibits the growth of bacteria (particularly Pseudomonas aeruginosa) and fungi (including Candida and Aspergillus species), acting as both a direct antimicrobial and a restorer of the ear canal's natural self-defence environment.
Dosages & Administration
The correct dose depends on the type and severity of infection, age, weight, and kidney function. Always follow your clinician's instructions. The table below is for general reference only.
Administration Tips
Shake the spray before use to ensure the suspension is uniformly mixed before each application.
Warm the spray by holding it in your hand for a few minutes before use to prevent the dizziness that cold solutions can cause in the ear canal.
Administration technique:
- Tilt your head to one side with the affected ear facing upward, or lie on your side.
- Pull the outer ear gently backward and upward (adults) to straighten the ear canal.
- Insert the spray nozzle carefully into the entrance of the ear canal — do not insert deeply.
- Press the actuator once to deliver one metered spray.
- Gently press the tragus (the small cartilage flap in front of the ear canal) several times to distribute the spray throughout the canal.
- Remain in position for 60 seconds before sitting upright.
Complete the full 7-day course even if symptoms improve earlier. Early cessation risks incomplete eradication of bacteria and recurrence.
Do not use if the eardrum is perforated — the neomycin component is ototoxic if it reaches the middle ear.
Side Effects of Otomize
Otomize is applied topically with minimal systemic absorption. Side effects are predominantly local.
- Transient burning or stinging on application
- Pruritus in the ear canal
- Ear discomfort
- Neomycin contact sensitivity / allergic contact dermatitis — localised allergic reaction to neomycin; presents as increased redness, swelling, and itching of the ear canal and surrounding skin; can occur even after previous uneventful use; requires immediate discontinuation
- Ototoxicity — hearing loss or balance disturbance if neomycin reaches the middle ear through an unrecognised eardrum perforation; irreversible cochlear or vestibular damage
- Fungal superinfection (otomycosis) — prolonged corticosteroid-containing ear preparations can promote fungal overgrowth; worsening symptoms or a change in discharge character should prompt clinical review
- Hypersensitivity reactions — rare generalised allergic reactions
Neomycin is one of the most common causes of allergic contact dermatitis from topical preparations. If you develop increased irritation, swelling, or a spreading rash around the ear during treatment — rather than improvement — this may indicate neomycin sensitisation. Stop Otomize immediately and contact your clinician. A patch test may be required to confirm neomycin allergy, and an alternative preparation without neomycin will be needed.
Drug Interactions
Otomize is a topical preparation with negligible systemic absorption at recommended doses. Clinically significant systemic interactions are not expected. Avoid concurrent use of other ototopical preparations in the same ear without clinical guidance.
Concurrent use may dilute the spray or interfere with its effectiveness; spacing administration or avoiding combination is recommended unless prescribed.
Theoretical risk of additive ototoxicity, especially if the tympanic membrane is perforated, allowing increased absorption.
Additive steroid exposure is possible but clinically insignificant due to minimal systemic absorption.
May alter the ear canal environment and reduce the effectiveness of the spray if used simultaneously.
Important Warnings
Otomize must not be used if there is any possibility of eardrum perforation. Neomycin is ototoxic and can cause irreversible sensorineural hearing loss and vestibular damage if it penetrates the middle ear. Otomize is also contraindicated in patients who have had ear surgery involving the middle ear or mastoid, and in patients with tympanostomy tubes, unless specifically assessed by an ENT specialist.
Neomycin is a potent contact allergen. Approximately 8–15% of people using neomycin-containing preparations develop contact sensitisation. If your ear symptoms are worsening rather than improving during treatment, or if you develop a spreading rash around the ear, stop Otomize and seek clinical review.
Prolonged use of corticosteroid-containing ear preparations should be avoided due to the risk of secondary fungal infection (otomycosis) and local corticosteroid effects. Use for the shortest necessary duration and do not repeat courses without clinical assessment.
Systemic absorption from topical ear preparations is negligible. Otomize may be used during pregnancy and breastfeeding when clearly indicated, but as with all medications, use only under clinician advice.

Both are combination antibiotic/steroid ear preparations for otitis externa. The key differences are: Otomize contains dexamethasone, neomycin, and acetic acid; Cilodex contains dexamethasone and ciprofloxacin. Ciprofloxacin (in Cilodex) has superior activity against Pseudomonas aeruginosa and is considered safe through tympanostomy tubes. Otomize (containing neomycin) is contraindicated with perforations due to ototoxicity risk.
