How Does Levothyroxine Sodium Work?
Levothyroxine is a synthetic, chemically identical copy of thyroxine (T4), the main thyroid hormone produced naturally by the thyroid gland. After being absorbed through the gastrointestinal tract, it enters the bloodstream and is transported to tissues throughout the body.
Within cells, T4 is converted by enzymes called deiodinases into the more biologically active form, triiodothyronine (T3). T3 then enters the cell nucleus and binds to thyroid hormone receptors, influencing the transcription of genes that regulate metabolism, protein synthesis, cardiovascular function, growth, and brain development.
By restoring circulating T4 levels to normal, levothyroxine reduces the pituitary gland's output of thyroid-stimulating hormone (TSH) -- normalising the feedback loop that governs thyroid activity and relieving the symptoms of hypothyroidism. Side Effects of Levothyroxine Sodium

Drug Interactions

Both significantly reduce the absorption of levothyroxine when taken at the same time. Levothyroxine should be taken at least 4 hours before or after calcium or iron supplements to preserve its bioavailability.

Levothyroxine can enhance the anticoagulant effect of warfarin, increasing bleeding risk. Closer INR monitoring is recommended when starting, stopping, or adjusting levothyroxine in patients on anticoagulant therapy.

Important Warnings
Levothyroxine increases the heart's metabolic demands. In older patients or those with known coronary artery disease or heart failure, doses must be started very low and increased extremely gradually to avoid precipitating angina, arrhythmia, or myocardial infarction. A full cardiovascular assessment is essential before initiating treatment in at-risk individuals.
Starting levothyroxine in a patient with unrecognised adrenal insufficiency can precipitate a life-threatening adrenal crisis. If adrenal problems are suspected -- particularly in patients with pituitary disease or symptoms of cortisol deficiency -- cortisol levels must be assessed and treated before thyroid hormone therapy is commenced.
Long-term TSH suppression -- particularly in post-menopausal women -- is associated with reduced bone mineral density and an increased risk of osteoporosis and fragility fractures.
