What Conditions Is Lamotrigine Used For?

How Does Lamotrigine Work?
Lamotrigine exerts its antiepileptic and mood-stabilising effects through voltage-dependent blockade of neuronal sodium channels. Specifically, it preferentially binds to and stabilises the inactivated state of sodium channels — the state they adopt immediately after firing — prolonging their inactivation and reducing the frequency at which neurons can fire repetitive action potentials.
By reducing excessive, high-frequency neuronal firing without significantly impairing normal neuronal activity at resting or low firing rates, lamotrigine suppresses the pathological repetitive neuronal discharges that underlie seizures. This 'use-dependent' or 'state-dependent' blockade is characteristic of several antiepileptic drugs.
A secondary mechanism involves inhibition of the presynaptic release of glutamate — the principal excitatory neurotransmitter in the brain — particularly in limbic regions. By reducing glutamatergic neurotransmission, lamotrigine provides additional stabilisation of neuronal circuits involved in both seizure propagation and the mood dysregulation of bipolar disorder.
Unlike many other antiepileptics, lamotrigine does not primarily enhance GABAergic inhibition, and it has no significant effect on calcium channels or receptor-mediated neurotransmission at therapeutic doses.

Administration Tips

Side Effects of Lamotrigine

- Aseptic meningitis — severe headache, stiff neck, fever, photophobia, nausea; rare but reported
- Haemophagocytic lymphohistiocytosis (HLH) — rare, serious inflammatory syndrome presenting with fever, rash, organ dysfunction
- Blood dyscrasias — rare; thrombocytopenia, neutropenia, aplastic anaemia
- Suicidal ideation — antiepileptic drugs as a class carry a small increased risk; monitor closely
Any rash occurring during the first 8 weeks of lamotrigine treatment must be taken seriously and reported to your clinician immediately — even if it appears mild. Approximately 10% of patients develop a rash; the majority are benign, but a small proportion progress to severe, life-threatening reactions including Stevens-Johnson Syndrome. Do not wait and observe — contact your clinician the same day any rash appears. If the rash is associated with fever, blistering, mouth sores, or skin peeling, seek emergency care immediately.
Drug Interactions
Lamotrigine has a particularly complex interaction profile. Many drugs significantly alter its metabolism, requiring dose adjustments. Always disclose all medications to your clinician and pharmacist.
These drugs significantly accelerate lamotrigine metabolism via CYP450 induction, approximately halving plasma lamotrigine levels. Higher lamotrigine doses are required when used concurrently. If an enzyme in,,ducer is withdrawn, lamotrigine levels will rise and dose reduction will be necessary.
Oestrogen-containing contraceptives significantly accelerate lamotrigine metabolism (by up to 50%), reducing lamotrigine plasma levels and potentially compromising seizure or mood control. Lamotrigine dose may need to be increased when starting COCs and reduced again during pill-free intervals. This interaction has caused unexpected seizures in women with epilepsy. A non-hormonal contraceptive or a progestogen-only method is often recommended; discuss with your clinician.
