As with all preparations containing neomycin, this ear spray carries an absolute contraindication in patients with a perforated tympanic membrane or tympanostomy tubes (grommets). Neomycin is directly ototoxic to the cochlea if it reaches the middle ear, and its use in these circumstances risks irreversible sensorineural hearing loss.

This ear spray acts solely within the external auditory canal. It has no role in the treatment of otitis media and is absolutely contraindicated where tympanic membrane perforation is suspected. Patients with deep ear pain, significant hearing loss, middle ear discharge, or systemic symptoms require assessment to exclude middle ear pathology before any topical ear treatment is commenced.
How Does This Ear Spray Work?
The three active ingredients act through distinct and complementary mechanisms that together address the inflammatory, bacterial, and microenvironmental components of otitis externa:
Dexamethasone 0.1% (corticosteroid) Dexamethasone suppresses the local inflammatory response by inhibiting phospholipase A2 and blocking the arachidonic acid pathway, thereby reducing the synthesis of prostaglandins, leukotrienes, and inflammatory cytokines within the ear canal. This reduces oedema, erythema, pruritus, and pain — and reduces the canal swelling that may otherwise impair the penetration of the antimicrobial components to the site of infection.
Neomycin sulfate 0.5% (aminoglycoside antibiotic) Neomycin is a bactericidal agent active against the predominant causative organisms of otitis externa, including Pseudomonas aeruginosa, Staphylococcus aureus, Proteus mirabilis, and Escherichia coli. It binds irreversibly to the 30S bacterial ribosomal subunit, inducing misreading of messenger RNA and causing lethal disruption of bacterial protein synthesis.
Glacial acetic acid 2% (acidifying antimicrobial) The healthy external ear canal maintains a naturally acidic environment (pH approximately 6.0–6.5), which limits microbial colonisation. Otitis externa disrupts this protective acidity. Acetic acid restores the canal's normal pH, directly inhibiting the growth of bacteria and fungi — including Candida and Pseudomonas species — while enhancing neomycin's antimicrobial activity and discouraging re-establishment of infection following treatment.

Administration Tips
Confirm tympanic membrane integrity before use. This preparation must not be used where perforation is possible. If there is any doubt, seek clinical assessment before starting treatment.
Warm the bottle to body temperature by holding it in your palm for two to three minutes before use. Applying cold solutions to the ear can provoke brief vertigo through caloric stimulation of the vestibular system.
For spray formulations: tilt the affected ear upward, introduce the nozzle gently at the entrance to the canal, and administer one metered spray. Maintain the position for one to two minutes.
For drop formulations: tilt the affected ear upward, instil the prescribed number of drops, and remain in this position for three to five minutes. Gentle tragal pressure (pressing the small cartilage flap in front of the ear) inward and outward a few times helps move the solution deeper into the canal.
Do not insert cotton buds, cotton wool, or any objects into the ear canal during the treatment course — these remove the medication and introduce further risk of damage or infection.
Complete the full course even if symptoms improve before seven days. Premature cessation risks incomplete bacterial eradication and early relapse.
Do not use for longer than seven days without clinical review. Prolonged use of combined antibiotic-corticosteroid ear preparations risks secondary fungal overgrowth and contact sensitisation to neomycin.
Side Effects of Dexamethasone / Neomycin / Acetic Acid Ear Spray
Most side effects are mild and resolve on their own. Serious side effects are rare but require immediate medical attention.
- Transient stinging or burning in the ear canal immediately after administration — brief and self-resolving
- Mild local itching or irritation within the canal
- Temporary sensation of fullness or muffled hearing immediately following administration
- Ototoxicity — neomycin is cochleotoxic and vestibulotoxic if it reaches the middle ear through an undetected or undisclosed perforation; new or worsening hearing loss, tinnitus, or vertigo during treatment requires immediate discontinuation and urgent clinical review
- Allergic contact dermatitis to neomycin — neomycin is one of the most common topical contact allergens; worsening rather than improving symptoms, spreading redness, swelling, or vesiculation may indicate hypersensitivity; discontinue and seek review
- Otomycosis (secondary fungal infection) — prolonged antibiotic use within the ear canal can disrupt the normal flora and allow overgrowth of Candida or Aspergillus species; new or worsening symptoms after initial improvement, particularly with thick or discoloured discharge, should prompt reassessment
- Local skin atrophy — rare; associated with prolonged or frequently repeated topical corticosteroid application within the ear canal
This preparation is absolutely contraindicated in patients with a perforated tympanic membrane, tympanostomy tubes (grommets), or any prior ear surgery that may have compromised membrane integrity. Direct cochlear exposure to neomycin can cause permanent, irreversible sensorineural hearing loss. Tympanic membrane integrity must be confirmed as intact before every course of this preparation.
Drug Interactions
Systemic absorption through an intact tympanic membrane is negligible, and clinically significant systemic drug interactions are rare with correct use. The following are nonetheless relevant
Concurrent systemic aminoglycoside therapy increases the aggregate ototoxic load. Neomycin-containing ear preparations should not be used alongside systemic aminoglycosides without specialist guidance.
Systemic loop diuretics potentiate aminoglycoside ototoxicity. Where tympanic membrane integrity is uncertain, this combination should be avoided.
Using multiple ear preparations concurrently is not recommended without clinical direction due to potential incompatibility between vehicles and active ingredients.
Where secondary fungal otitis externa develops during or following a treatment course, clinician-directed topical antifungal therapy will typically be required as a separate prescription rather than continuation of this preparation.
Important Warnings
This preparation is absolutely contraindicated in patients with a known or suspected perforated eardrum, grommets, or prior middle ear surgery affecting membrane integrity. Neomycin is directly toxic to the cochlea and can cause permanent, irreversible hearing damage if it reaches the middle ear. This must be confirmed absent before prescribing or using this preparation.
Patients with a known allergy to neomycin or any other aminoglycoside antibiotic must not use this preparation. Cross-reactivity within the aminoglycoside class (neomycin, gentamicin, tobramycin, framycetin, streptomycin) is well documented. Worsening of symptoms after commencing treatment may represent a hypersensitivity reaction rather than treatment failure; clinical review is essential if this occurs.
This preparation should not be used for more than seven days without reassessment by a clinician. Patients who do not show clear improvement within this period require review to confirm the diagnosis, exclude resistant organisms, and assess for alternative pathology.
This preparation is not recommended for children under two years of age. In young children, or where reliable assessment of tympanic membrane integrity is not possible, ENT or paediatric specialist review should be sought before initiating treatment.
Systemic absorption from correctly used topical ear preparations is minimal. However, use during pregnancy and breastfeeding should be limited to situations where the clinical benefit clearly outweighs any theoretical risk, and only following clinician assessment.
