What Conditions Is Desloratadine Used For?
Desloratadine is prescribed for allergic conditions driven by histamine release:
relief of sneezing, itchy and runny nose, nasal congestion, and itchy, watery eyes caused by seasonal allergens such as grass, tree, and weed pollens.
year-round nasal and eye allergy symptoms caused by persistent allergens such as house dust mites, pet dander, and mould spores.
relief of itching and reduction of hives (raised, red, itchy welts on the skin) in patients with chronic idiopathic urticaria lasting six weeks or more.
itching, redness, and watering of the eyes associated with allergic rhinitis.

Desloratadine is not suitable for treating severe allergic reactions (anaphylaxis). If you experience sudden difficulty breathing, swelling of the face or throat, a rapid drop in blood pressure, or collapse following allergen exposure, call 999 immediately. Anaphylaxis requires emergency treatment with adrenaline (epinephrine).
How Does Desloratadine Work?
When the immune system encounters an allergen it has been sensitised to, mast cells and basophils release histamine – a chemical messenger that binds to histamine H1 receptors throughout the body. This binding triggers the familiar symptoms of an allergic reaction: sneezing, itching, nasal congestion, watery eyes, and skin reactions.
Desloratadine works by selectively and competitively blocking peripheral H1 histamine receptors, preventing histamine from binding and thereby inhibiting the allergic response at its source. Because it binds to receptors without activating them, desloratadine is classified as an inverse agonist rather than a simple antagonist – meaning it actively reduces baseline receptor activity as well as blocking histamine's effects.
A key advantage of desloratadine over first-generation antihistamines is its low ability to cross the blood-brain barrier. Central nervous system H1 receptors are involved in maintaining wakefulness, so older antihistamines that enter the brain readily cause sedation. Desloratadine's selective peripheral action means it relieves allergy symptoms with significantly less drowsiness, making it suitable for daytime use and safe to take while driving or operating machinery in most patients.

Administration Tips

Side Effects of Desloratadine
Desloratadine is generally very well tolerated. Most side effects are mild and uncommon. Serious adverse effects are rare.

Although desloratadine is classified as non-sedating, a small number of individuals may experience drowsiness, particularly at the start of treatment. Until you know how desloratadine affects you personally, exercise caution when driving or operating heavy machinery.
Drug Interactions
Always disclose all prescription drugs, over-the-counter medicines, vitamins, and supplements to your clinician or pharmacist before starting desloratadine.
Although desloratadine is non-sedating for most people, combining it with alcohol may increase the risk of drowsiness and impaired reaction times. Avoid alcohol or consume it only in moderation during treatment.
Concurrent use of desloratadine with central nervous system depressants such as benzodiazepines, sleeping tablets, or certain pain medications may increase sedative effects. Use with caution and inform your clinician of all medications you are taking.
Studies have shown that erythromycin (an antibiotic) and ketoconazole (an antifungal) can increase blood levels of desloratadine. Although this has not been shown to cause clinically significant effects at standard doses, it is worth informing your clinician if you are taking these medications.
This stomach acid medication can increase plasma concentrations of desloratadine. No dose adjustment is generally required, but inform your clinician or pharmacist if you take cimetidine regularly.
Using desloratadine alongside other antihistamines – including over-the-counter products – increases the risk of side effects without providing additional benefit. Avoid taking multiple antihistamines simultaneously unless directed by a clinician.
Important Warnings
Desloratadine is not appropriate for treating anaphylaxis or severe allergic emergencies. If you are at risk of anaphylaxis, your clinician will prescribe an adrenaline auto-injector (such as an EpiPen) as your primary emergency treatment. Desloratadine may be taken alongside this for ongoing symptom management but cannot replace emergency treatment.
Some formulations of desloratadine oral solution contain phenylalanine. Patients with phenylketonuria (PKU) must check the product formulation with their pharmacist before use.
Patients with significantly reduced kidney function should use desloratadine with caution and may require dose adjustment. Your clinician will assess your kidney function and prescribe an appropriate dose.
There is limited data on the safety of desloratadine during pregnancy. Animal studies have not shown harmful effects, but as a precaution, desloratadine should only be used during pregnancy if clearly necessary and under clinician supervision. Desloratadine passes into breast milk and is generally not recommended during breastfeeding. A clinician may recommend an alternative antihistamine considered safer during this period.
Older patients may be more sensitive to antihistamines. Although desloratadine is less sedating than first-generation antihistamines, any drowsiness or dizziness experienced by elderly patients should be taken seriously given the increased risk of falls.
