Current guidance from the Medicines and Healthcare products Regulatory Agency (MHRA) recommends that ciprofloxacin should be reserved for conditions where other antibiotics are not suitable.
Ciprofloxacin is available in tablet, oral suspension, and intravenous infusion forms, as well as topical preparations for ear and eye infections. It is suitable for adults and, in specific circumstances, children. It is available generically and under brand names including Ciproxin.

What Conditions Is Ciprofloxacin Used For?
Ciprofloxacin is prescribed for a range of bacterial infections, typically where first-line antibiotics are unsuitable or have failed:
complicated UTIs and pyelonephritis (kidney infections) caused by susceptible organisms; used when first-line agents such as trimethoprim or nitrofurantoin are inappropriate.

Due to concerns about serious side effects and antibiotic resistance, UK prescribing guidelines recommend that ciprofloxacin should not be used as a first-line antibiotic for most common infections where alternative agents are available. How Does Ciprofloxacin Work?
Ciprofloxacin belongs to the fluoroquinolone class of antibiotics and exerts its bactericidal effects by targeting two essential bacterial enzymes involved in DNA replication and repair: DNA gyrase (topoisomerase II) and topoisomerase IV.
These enzymes are responsible for controlling the supercoiling and unwinding of bacterial DNA during replication and transcription – processes that are essential for bacteria to copy their genetic material and produce the proteins needed for growth and survival. Ciprofloxacin forms stable complexes with these enzymes and the bacterial DNA strand, effectively trapping the enzymes in a state that causes irreparable double-strand breaks in the DNA. This prevents the bacteria from replicating their DNA, halts protein synthesis, and ultimately causes bacterial cell death.
Because ciprofloxacin directly destroys bacterial DNA, it is bactericidal – it kills bacteria outright rather than simply inhibiting their growth. Its mechanism is entirely different from beta-lactam antibiotics (which target cell walls) and macrolides or tetracyclines (which target ribosomes), which is why it is effective against many organisms that are resistant to these other antibiotic classes.
Ciprofloxacin has excellent oral bioavailability (approximately 70–80%), meaning that oral doses achieve blood levels close to those achieved by intravenous administration. It also achieves very high concentrations in many tissues – including the prostate, bone, lung, and kidneys – making it effective for infections in these sites. Its activity is predominantly against Gram-negative bacteria, though it also covers some Gram-positive and atypical organisms.

Administration Tips
Ciprofloxacin tablets can be taken with or without food. Food does not significantly affect absorption, though taking with food may reduce nausea. Avoid taking with dairy products or calcium-fortified foods immediately before or after the dose, as calcium can reduce absorption.
Avoid taking ciprofloxacin within 2 hours before or 6 hours after antacids containing aluminium, magnesium, or calcium; iron supplements; zinc supplements; or sucralfate. These products bind to ciprofloxacin in the gut, significantly reducing its absorption and effectiveness.
Maintain adequate hydration throughout treatment. Ciprofloxacin can rarely cause crystalluria (crystals in the urine), and drinking sufficient water reduces this risk and supports kidney function.
Avoid excessive caffeine during ciprofloxacin treatment, as ciprofloxacin inhibits caffeine metabolism, which can lead to caffeine accumulation and cause agitation, palpitations, and insomnia.
Avoid prolonged sun exposure and sunbeds during treatment. Ciprofloxacin can cause photosensitivity, increasing the risk of sunburn. Use high-SPF sunscreen and protective clothing when outdoors.
Stop ciprofloxacin immediately and rest the affected area if you experience any pain, swelling, or inflammation in a tendon, particularly the Achilles tendon. Contact your clinician promptly. Do not wait to see if it resolves.
Always complete the full prescribed course even if symptoms improve early, to ensure the infection is fully eradicated and reduce the risk of resistance.
Side Effects of Ciprofloxacin
Ciprofloxacin has a well-documented safety profile that includes some serious side effects that are not commonly associated with other antibiotic classes. Patients should be made aware of these before starting treatment.

- Tendinopathy and tendon rupture – pain, swelling, or rupture of tendons, most commonly the Achilles tendon; can occur during treatment or up to several months after; risk is substantially higher in patients over 60, those on corticosteroids, and those with kidney disease
- Peripheral neuropathy – numbness, tingling, burning, or weakness in the limbs; can be irreversible; stop ciprofloxacin immediately if these symptoms develop
- Central nervous system effects – seizures, confusion, hallucinations, agitation, anxiety, depression, insomnia, and psychosis; can occur even on the first dose
- Cardiac arrhythmias (QT prolongation) – potentially fatal heart rhythm disturbances, particularly in patients with pre-existing cardiac conditions or those taking other QT-prolonging drugs
- Aortic aneurysm and dissection – rare but serious; ciprofloxacin has been associated with weakening of the aortic wall; avoid in patients with known aortic aneurysm or risk factors
- C. difficile-associated diarrhoea – persistent watery or bloody diarrhoea, abdominal cramping, fever
- Severe allergic reaction (anaphylaxis) – hives, facial or throat swelling, difficulty breathing, collapse
- Hepatotoxicity – jaundice, dark urine, pale stools, severe fatigue; rare
- Blood sugar disturbances – both hypoglycaemia and hyperglycaemia have been reported, particularly in diabetic patients on antidiabetic medications
- Myasthenia gravis exacerbation – ciprofloxacin can worsen neuromuscular blockade in patients with myasthenia gravis
The UK Medicines and Healthcare products Regulatory Agency (MHRA) has issued guidance highlighting that fluoroquinolone antibiotics including ciprofloxacin are associated with rare but disabling and potentially irreversible side effects affecting tendons, muscles, joints, nerves, and the central nervous system. These reactions have been reported to occur together as a syndrome in some patients. Ciprofloxacin should be discontinued at the first sign of any serious adverse reaction and should not be prescribed for mild or self-limiting infections where other options are available.
Drug Interactions

Combining ciprofloxacin with other QT-prolonging medications significantly increases the risk of serious, potentially fatal cardiac arrhythmias. This combination should generally be avoided or used only under close cardiac monitoring.
Ciprofloxacin dramatically increases plasma levels of tizanidine (a muscle relaxant) by inhibiting its metabolism, causing severe hypotension and sedation. This combination is contraindicated.

Important Warnings
Ciprofloxacin and other fluoroquinolones are associated with tendinitis (tendon inflammation) and tendon rupture, most commonly affecting the Achilles tendon. Rupture can occur during treatment or up to several months after stopping. Risk is substantially higher in patients over 60, those receiving concurrent corticosteroid treatment, patients with kidney disease, and those who have undergone organ transplantation. Ciprofloxacin can cause peripheral neuropathy – damage to the nerves of the limbs – presenting as burning, tingling, numbness, or weakness. This can develop rapidly after starting treatment and may be irreversible if ciprofloxacin is not stopped promptly. Serious psychiatric and neurological effects including seizures, confusion, hallucinations, depression, anxiety, and psychosis have been reported with ciprofloxacin, sometimes on the first dose. Ciprofloxacin has been associated with an increased risk of aortic aneurysm and aortic dissection – rare but potentially fatal conditions caused by weakening of the aortic wall. Ciprofloxacin should be used with extreme caution or avoided in patients with known aortic aneurysm, risk factors for aortic disease (hypertension, atherosclerosis, Marfan syndrome), or a family history of aortic disease. Seek emergency medical attention if you develop sudden severe chest, back, or abdominal pain during ciprofloxacin treatment
Ciprofloxacin prolongs the cardiac QT interval and can trigger serious arrhythmias. Risk is highest in patients with pre-existing cardiac conditions, electrolyte abnormalities, or those taking other QT-prolonging medications. Always disclose your full cardiac and medication history before being prescribed ciprofloxacin.
Ciprofloxacin is one of the antibiotics most commonly associated with Clostridioides difficile infection. If you develop persistent, watery, or bloody diarrhoea during or after treatment, contact a clinician promptly and do not self-treat with anti-diarrhoeal medications.
Ciprofloxacin is generally not recommended during pregnancy as it may affect developing cartilage and joints in the foetus, based on animal studies.
