Celecoxib is a selective cyclooxygenase-2 (COX-2) inhibitor, a class of non-steroidal anti-inflammatory drug (NSAID) known as a coxib. It provides anti-inflammatory, analgesic (pain-relieving), and antipyretic (fever-reducing) effects while carrying a significantly reduced risk of gastrointestinal side effects compared to traditional, non-selective NSAIDs such as ibuprofen or naproxen.
First approved in the late 1990s, celecoxib represented a significant advance in pain management for patients requiring long-term NSAID therapy, particularly those at risk of peptic ulcers or gastrointestinal bleeding. By selectively targeting COX-2 — the enzyme responsible for producing prostaglandins at sites of inflammation — while sparing COX-1 (which protects the gastric lining), celecoxib achieves effective pain relief with a more favourable gastrointestinal safety profile.
Celecoxib is available in capsule form and is suitable for adults. It is indicated for chronic inflammatory conditions such as arthritis, for acute pain and dysmenorrhoea, and in a specialist setting for the management of familial adenomatous polyposis (FAP).

What Conditions Is Celecoxib Used For?
Celecoxib is prescribed for a range of inflammatory and painful conditions:
pain and inflammation affecting joints caused by cartilage degeneration.
chronic autoimmune joint inflammation causing pain, swelling, and stiffness.
inflammatory arthritis primarily affecting the spine.
short-term management of pain following injury, surgery, or dental procedures.
painful periods caused by uterine cramping.
used adjunctively to reduce the number of adenomatous colorectal polyps in patients with FAP.
Celecoxib relieves pain and inflammation but does not alter the underlying disease process of arthritis or other inflammatory conditions. It should be used at the lowest effective dose for the shortest necessary duration to manage symptoms.
How Does Celecoxib Work?
Celecoxib selectively inhibits the cyclooxygenase-2 (COX-2) enzyme, which is responsible for converting arachidonic acid into prostaglandins — lipid mediators that sensitise pain receptors, promote inflammation, and cause fever at sites of tissue injury and inflammation.
By blocking COX-2 specifically, celecoxib reduces prostaglandin production at sites of inflammation, relieving pain, swelling, and fever. Crucially, celecoxib does not significantly inhibit COX-1 at therapeutic doses. COX-1 is constitutively expressed in the gastric mucosa, kidneys, and platelets, where it produces prostaglandins that protect the stomach lining, maintain renal blood flow, and support normal platelet aggregation. Sparing COX-1 accounts for celecoxib's improved gastrointestinal tolerability relative to non-selective NSAIDs.
However, COX-2 inhibition also reduces prostacyclin production in blood vessels (a vasodilator and platelet aggregation inhibitor), while leaving thromboxane A2 production (a platelet activator) relatively unaffected — creating a prothrombotic imbalance that underlies the cardiovascular risks associated with COX-2 selective inhibitors.

Administration Tips
Celecoxib can be taken with or without food. Taking it with food may reduce the risk of gastrointestinal upset, particularly in sensitive individuals.
Capsules may be opened and the contents mixed with applesauce if swallowing whole is difficult — the mixture should be swallowed immediately without chewing.
Use the lowest effective dose for the shortest duration necessary to control symptoms, particularly in patients with cardiovascular risk factors.
Do not take other NSAIDs (including over-the-counter ibuprofen or aspirin at anti-inflammatory doses) while taking celecoxib — combining NSAIDs increases gastrointestinal and cardiovascular risks without additional therapeutic benefit.
Low-dose aspirin (75 mg) for cardiovascular protection may be continued alongside celecoxib, but it increases gastrointestinal risk; consider gastroprotection (e.g. a proton pump inhibitor) if this combination is required.
Side Effects of Celecoxib
Celecoxib is generally well tolerated, particularly with respect to gastrointestinal side effects compared to non-selective NSAIDs. However, significant risks exist, particularly cardiovascular.

- Dizziness
- Insomnia
- Cardiovascular events — myocardial infarction (heart attack) and stroke; risk is increased with long-term use and higher doses, particularly in patients with pre-existing cardiovascular disease
- Serious gastrointestinal events — upper GI ulceration, bleeding, or perforation; less common than with non-selective NSAIDs but still possible
- Renal impairment — fluid retention, worsening hypertension, acute kidney injury; risk increased with concurrent diuretics, ACE inhibitors, or existing renal disease
- Hepatotoxicity — elevated liver enzymes, jaundice, hepatic failure (rare)
- Severe skin reactions — Stevens-Johnson Syndrome, Toxic Epidermal Necrolysis; rare but serious
- Anaphylaxis — rare; risk increased in patients allergic to sulfonamides
All COX-2 inhibitors, including celecoxib, are associated with an increased risk of cardiovascular events such as heart attack and stroke, particularly with long-term use and at higher doses. This risk is greatest in patients with established cardiovascular disease. Celecoxib should be used with caution in patients with hypertension, hyperlipidaemia, diabetes, or a history of cardiac events, and should be avoided in patients who have recently had coronary artery bypass graft surgery.
Drug Interactions
Always disclose all prescription drugs, over-the-counter medicines, vitamins, and supplements to your clinician or pharmacist before starting celecoxib.
Celecoxib can enhance the anticoagulant effect, increasing the risk of bleeding. INR monitoring is required when initiating or changing celecoxib dose in patients on warfarin.
Significantly increases celecoxib plasma concentrations; the celecoxib dose should be halved when used concurrently.
Celecoxib may reduce the antihypertensive effect and increase the risk of acute renal impairment, particularly in volume-depleted or elderly patients.
NSAIDs including celecoxib can increase lithium plasma concentrations by reducing renal lithium excretion; lithium levels should be monitored.
Important Warnings
Celecoxib is contraindicated in patients with established ischaemic heart disease, peripheral arterial disease, cerebrovascular disease (including stroke), or in patients who have undergone coronary artery bypass grafting. In patients with risk factors, the lowest effective dose should be used for the shortest possible time.
Although less common than with non-selective NSAIDs, gastrointestinal ulceration, bleeding, and perforation can occur with celecoxib. Risk is increased in the elderly, those with a history of peptic ulcer disease, heavy alcohol users, and those on concurrent anticoagulants or corticosteroids. Gastroprotection with a proton pump inhibitor should be considered in at-risk patients.
Celecoxib contains a sulfonamide group and may cause reactions in patients with a known sulfonamide allergy. Inform your clinician of any drug allergies before starting treatment.
Celecoxib can cause fluid retention, oedema, and worsening of hypertension. Blood pressure should be monitored regularly during treatment.

Yes, in general. Because celecoxib selectively inhibits COX-2 while largely sparing COX-1 (which protects the stomach lining), it causes significantly fewer gastrointestinal side effects such as ulcers and bleeding compared to non-selective NSAIDs like ibuprofen or naproxen. However, gastrointestinal events can still occur, particularly in older patients or those with pre-existing gut conditions.
Like other NSAIDs, celecoxib can cause fluid retention and a modest rise in blood pressure. If you have hypertension, your blood pressure should be monitored during treatment and your antihypertensive medications reviewed if necessary.
Low-dose aspirin (75 mg) for cardiovascular protection can generally be continued alongside celecoxib, but the combination increases gastrointestinal risk. Your clinician may recommend a proton pump inhibitor (PPI) for stomach protection if you require both medications.
