What Is Azithromycin?
Azithromycin is a broad-spectrum macrolide antibiotic used to treat a wide range of bacterial infections. It is one of the most widely prescribed antibiotics in the world and is particularly valued for its convenience – many courses consist of just 3 to 5 days of treatment, or in some cases a single dose, due to the drug's exceptionally long half-life and its ability to accumulate at the site of infection.
Developed in the 1980s as a derivative of erythromycin, azithromycin was designed to overcome some of erythromycin's limitations, including its short duration of action, frequent dosing requirements, and poor gastrointestinal tolerability. Azithromycin offers improved tissue penetration, a longer duration of action, and a more convenient dosing schedule, while maintaining a broad spectrum of antibacterial activity.
It is particularly effective against atypical organisms – bacteria that cannot be targeted by standard penicillin antibiotics – making it a valuable alternative in penicillin-allergic patients and a first-line treatment for certain respiratory, sexually transmitted, and skin infections. Azithromycin is available in tablet, capsule, and oral suspension forms and is suitable for adults and children.

What Conditions Is Azithromycin Used For?
Azithromycin is prescribed for a variety of bacterial infections across multiple body systems:
community-acquired pneumonia, including atypical pneumonia caused by Mycoplasma pneumoniae, Chlamydophila pneumoniae, and Legionella pneumophila; acute bronchitis in patients where an antibiotic is warranted.
streptococcal pharyngitis and tonsillitis, particularly in patients allergic to penicillin.
acute otitis media in adults and children where first-line treatment has failed or is unsuitable.
acute bacterial sinusitis in penicillin-allergic patients or where first-line antibiotics are inappropriate.

Azithromycin is an antibiotic and has no effect on viruses. It will not treat the common cold, influenza, COVID-19, or other viral illnesses. Prescribing antibiotics for viral infections contributes to antibiotic resistance and should be avoided.
How Does Azithromycin Work?
Azithromycin belongs to the macrolide class of antibiotics and works by inhibiting bacterial protein synthesis. It binds to the 50S subunit of the bacterial ribosome – the cellular machinery responsible for constructing proteins – and blocks the process of translocation, which is the movement of the ribosome along the messenger RNA strand during protein production.
By preventing protein synthesis, azithromycin stops bacteria from producing the enzymes, structural proteins, and regulatory molecules they need to grow and replicate. At standard doses, this results in a bacteriostatic effect – inhibiting bacterial growth and allowing the immune system to clear the infection. At higher concentrations achieved at infection sites due to azithromycin's exceptional tissue penetration, it may exert bactericidal effects against some organisms.
One of azithromycin's most distinctive pharmacological features is its remarkable ability to accumulate within cells – particularly within phagocytes (immune cells such as macrophages and neutrophils) that migrate to sites of infection. This results in drug concentrations at infected tissues that are 10 to 100 times higher than blood plasma levels, allowing it to target intracellular pathogens such as Chlamydia, Mycoplasma, and Legionella that reside inside host cells and are inaccessible to many other antibiotics.
Azithromycin also has anti-inflammatory properties independent of its antibacterial activity, modulating immune responses in ways that may contribute to its clinical effectiveness in certain respiratory conditions.

Administration Tips
Azithromycin tablets can be taken with or without food. If you experience nausea, taking it with a light meal may help. However, azithromycin capsules should be taken on an empty stomach, at least 1 hour before or 2 hours after food, as food reduces their absorption.
Take at the same time each day to maintain consistent levels and make it easier to remember, especially for short 3-day or 5-day courses.
Do not take antacids containing aluminium or magnesium at the same time as azithromycin, as these can reduce its absorption. Space doses at least 2 hours apart from antacid use.
Complete the full prescribed course even if symptoms improve after just 1 or 2 days. Azithromycin continues to work for several days after the last dose due to its long half-life, but stopping early can still allow bacteria to survive and return.
Store oral suspension in the refrigerator after reconstitution and discard any unused portion after 10 days.
Side Effects of Azithromycin
Azithromycin is generally well tolerated. Most side effects are gastrointestinal and mild. Serious side effects are uncommon but require prompt medical attention.

- Cardiac arrhythmias (QT prolongation) – azithromycin can affect the heart's electrical conduction and cause potentially serious irregular heart rhythms, particularly in patients with pre-existing heart conditions or those taking other QT-prolonging medications
- Severe allergic reaction (anaphylaxis) – sudden onset hives, facial or throat swelling, difficulty breathing, collapse
- Severe skin reactions – Stevens-Johnson Syndrome or Toxic Epidermal Necrolysis; blistering, widespread peeling skin
- Liver toxicity – jaundice, dark urine, pale stools, severe fatigue; rare but reported
- C. difficile-associated diarrhoea – persistent watery or bloody diarrhoea, cramping, fever
- Hearing loss or tinnitus – rare; more likely with high doses or prolonged treatment
Azithromycin has a well-documented association with QT interval prolongation, which can lead to a serious and potentially fatal heart arrhythmia called Torsades de Pointes. Risk is highest in patients with a history of cardiac arrhythmias, congenital long QT syndrome, low potassium or magnesium levels, bradycardia, or those taking other QT-prolonging drugs. Always disclose your full cardiac history and current medications before being prescribed azithromycin.
Drug Interactions
Always disclose all prescription drugs, over-the-counter medicines, vitamins, and supplements to your clinician or pharmacist before starting Azithromycin.
Combining azithromycin with other medications that prolong the QT interval significantly increases the risk of dangerous cardiac arrhythmias. This combination should generally be avoided. Inform your clinician of all heart or psychiatric medications you are taking.
Azithromycin may increase blood levels of ergot alkaloids, potentially causing ergotism – severe vasoconstriction leading to limb ischaemia. Concurrent use is contraindicated.
Azithromycin may enhance the anticoagulant effect of warfarin, increasing the risk of bleeding. INR should be monitored closely during and after treatment in patients taking warfarin.
Azithromycin may increase digoxin absorption by altering gut bacteria, raising digoxin blood levels and the risk of toxicity. Digoxin levels should be monitored during concurrent use.
Azithromycin may increase ciclosporin plasma concentrations, raising the risk of nephrotoxicity and other ciclosporin-related adverse effects. Monitoring is required.
Concurrent use reduces azithromycin absorption. Space doses at least 2 hours apart from antacid use.
As with other antibiotics, there is a theoretical risk of reduced contraceptive efficacy. Use additional contraceptive precautions during treatment.
Nelfinavir may increase azithromycin blood levels, potentially increasing the risk of side effects. Monitoring is advisable in patients receiving HIV treatment.
Important Warnings
Azithromycin has been associated with cases of potentially fatal cardiac arrhythmias due to QT interval prolongation. Patients with known cardiac conditions, electrolyte abnormalities, or those taking other QT-prolonging medications are at highest risk. Seek immediate medical attention if you experience palpitations, irregular heartbeat, fainting, or dizziness during treatment.
Serious hypersensitivity reactions including anaphylaxis and severe skin reactions (Stevens-Johnson Syndrome) have been reported with azithromycin. Some reactions have recurred following re-exposure despite initial cessation. If a severe reaction occurs, do not re-take azithromycin without specialist allergy assessment.
Like all antibiotics, azithromycin can disrupt gut flora and allow Clostridioides difficile to overgrow. If persistent, watery, or bloody diarrhoea develops during or after treatment, contact a clinician promptly. Do not use anti-diarrhoeal medications without medical advice in this context.
Azithromycin is metabolised by the liver and excreted primarily in bile. Patients with pre-existing liver disease should use it with caution and under close medical supervision. Liver function should be monitored in patients on prolonged courses.
Azithromycin has been reported to exacerbate symptoms of myasthenia gravis – a neuromuscular condition causing muscle weakness. Patients with this condition should inform their clinician before starting azithromycin, as alternative antibiotics may be more appropriate.
Azithromycin is generally considered acceptable for use during pregnancy when the benefits outweigh the risks, and it is sometimes used for the treatment of chlamydia in pregnancy. It passes into breast milk in small amounts. Consult your clinician before starting azithromycin if you are pregnant or breastfeeding.

Hyoscine butylbromide can be taken regularly for IBS management — some patients take it before meals to prevent spasm. It is generally considered safe for regular use, but persistent or worsening symptoms should be reviewed by a clinician to ensure the diagnosis is correct and that other contributing factors are addressed. Long-term reliance on any antispasmodic without addressing dietary and lifestyle factors is not ideal.
Azithromycin is rapidly absorbed and begins working within a few hours of the first dose. Most patients notice an improvement in symptoms within 24 to 48 hours. Importantly, azithromycin continues to work for several days after the last dose is taken, due to its long half-life and high tissue concentrations.
Azithromycin has an exceptionally long half-life of approximately 68 hours and accumulates at very high concentrations in infected tissues. This means that even after a 3 or 5-day course, therapeutic drug levels remain at the site of infection for several more days, continuing to treat the infection. Yes. A single 1 g dose of azithromycin is a recognised treatment for uncomplicated genital chlamydia infection. However, doxycycline (100 mg twice daily for 7 days) is now the preferred first-line treatment in current UK guidelines due to slightly superior efficacy. After an oral dose, hyoscine butylbromide typically begins to relieve cramping and spasm within 15 to 30 minutes, though its poor oral bioavailability means the effect may be more modest than with the intramuscular or intravenous route. Injected forms work within minutes and are used for rapid spasm relief in hospital settings.
There is no known dangerous interaction between alcohol and azithromycin. However, alcohol can suppress the immune system and slow recovery from infection. It is generally advisable to limit alcohol consumption while unwell and taking antibiotics.
Azithromycin is associated with a small risk of QT interval prolongation – an electrical change in the heart that can predispose to arrhythmias. This risk is most relevant in patients with pre-existing heart conditions, low electrolyte levels, or those taking other QT-prolonging medications.
