What Is Amitriptyline?
Amitriptyline is a tricyclic antidepressant (TCA) that has been in clinical use since the 1960s. While it was originally developed to treat depression, it is now used far more commonly at low doses for a wide range of other conditions -- including chronic pain, nerve pain, migraines, and sleep disturbance -- than it is for depression itself.
At low doses (typically 10--50 mg), amitriptyline has analgesic and sedative properties that are distinct from its antidepressant effects. This makes it a versatile and well-established option for patients with conditions such as fibromyalgia, neuropathic pain, tension headaches, and irritable bowel syndrome. At higher doses (75--150 mg or above), it is used as an antidepressant, though it has largely been replaced in this role by newer, better-tolerated medications such as SSRIs.
Amitriptyline is available as tablets and oral solution and is suitable for adults. It is available generically and under the brand name Tryptizol.

What Conditions Is Amitriptyline Used For?
Amitriptyline is used across a broad range of conditions, many of which are off-label indications supported by substantial clinical evidence:

How Does Amitriptyline Work?
Amitriptyline's therapeutic effects arise from several distinct pharmacological actions, which vary in relevance depending on the dose and the condition being treated.
As an antidepressant, amitriptyline works primarily by inhibiting the reuptake of serotonin and noradrenaline (norepinephrine) in the brain. By blocking the transporter proteins that remove these neurotransmitters from the synaptic cleft, amitriptyline increases their availability and prolongs their signalling effects. Elevated serotonin and noradrenaline activity is associated with mood regulation, emotional resilience, and reduced anxiety, which underpins the antidepressant effect at higher doses.
As a pain modulator, amitriptyline's analgesic effects at low doses are thought to arise from a combination of enhanced noradrenergic and serotonergic signalling in descending pain pathways, which suppress pain transmission in the spinal cord, as well as sodium channel blockade in peripheral nerves, which reduces abnormal pain signalling. These effects are independent of any antidepressant action and occur at much lower doses.
As a sleep aid, amitriptyline has potent antihistamine (H1 receptor blocking) and anticholinergic properties that produce sedation. This is why even very low doses (10 mg) taken at night can significantly improve sleep quality -- but it also accounts for many of amitriptyline's common side effects such as dry mouth, constipation, and morning grogginess.
It also has sodium channel blocking activity in cardiac tissue, which explains the serious cardiac risks associated with overdose and the need for caution in patients with pre-existing heart conditions.

Administration Tips
Amitriptyline is almost always taken at night, shortly before bed. Its sedative properties make taking it during the day impractical for most people, and evening dosing also means the peak sedative effect occurs during sleep rather than during waking hours.
Start low and go slow. Doses are typically started at the lowest effective level and increased gradually every 1--2 weeks as tolerated, to minimise side effects and allow the body to adjust.
Do not stop amitriptyline suddenly. Abrupt discontinuation can cause withdrawal symptoms including nausea, flu-like symptoms, insomnia, irritability, and dizziness. If stopping treatment, doses should be tapered gradually under your clinician's guidance.
Allow adequate time to assess effectiveness. For pain and migraine prevention, allow at least 6--8 weeks at a therapeutic dose before concluding whether treatment is working. For depression, allow 4--6 weeks.
Avoid alcohol during amitriptyline treatment. Alcohol significantly enhances the sedative effects and can increase the risk of falls, accidents, and impaired judgement.
Side Effects of Amitriptyline
Side effects are common with amitriptyline and are largely related to its anticholinergic and antihistaminergic properties. Most are more pronounced at higher doses and tend to improve over time as the body adjusts. At the low doses used for pain and migraine, side effects are often mild.
- Drowsiness or morning sedation (very common, especially at first)
- Dry mouth
- Constipation
- Blurred vision
- Urinary hesitancy or difficulty passing urine
- Weight gain
- Dizziness on standing (postural hypotension)
- Increased heart rate (tachycardia)
- Difficulty concentrating or mental "fogginess"
- Increased sweating
- Cardiac arrhythmias -- QT interval prolongation, heart block, and serious rhythm disturbances, particularly in overdose or in patients with pre-existing heart disease
- Suicidal thoughts or worsening depression -- particularly important in young adults and adolescents in the early weeks of treatment; requires close monitoring
- Serotonin syndrome -- when combined with other serotonergic drugs; presents as agitation, confusion, rapid heart rate, high temperature, and muscle rigidity
- Seizures -- amitriptyline lowers the seizure threshold; risk is higher at elevated doses or in those with a history of epilepsy
- Severe anticholinergic toxicity -- urinary retention, acute angle-closure glaucoma, severe confusion (particularly in elderly patients)
- Liver toxicity -- elevated liver enzymes and jaundice; rare but reported
Amitriptyline's anticholinergic and sedative properties make it potentially inappropriate for elderly patients, who are more sensitive to its effects and at higher risk of confusion, falls, urinary retention, and cardiac complications. It appears on the Beers Criteria list of medications to avoid in older adults. Clinicians will carefully weigh risks and benefits before prescribing in this age group.
Drug Interactions
Amitriptyline has a wide range of clinically important drug interactions. Always disclose all prescription drugs, over-the-counter medicines, vitamins, and supplements to your clinician or pharmacist before starting treatment.
Combining amitriptyline with MAOIs can cause a potentially fatal interaction including hypertensive crisis and severe serotonin syndrome. Amitriptyline must not be started within 14 days of stopping an MAOI, and an MAOI must not be started within 14 days of stopping amitriptyline.
Combining amitriptyline with other medications that prolong the QT interval increases the risk of serious cardiac arrhythmias. A full medication review is essential before prescribing.
Some SSRIs -- particularly fluoxetine and paroxetine -- inhibit the liver enzyme CYP2D6 that metabolises amitriptyline, causing its blood levels to rise significantly. This increases the risk of toxicity and serotonin syndrome. Dose adjustment and monitoring are required.
Concurrent use increases the risk of seizures and serotonin syndrome. This combination should be used with caution and only under close medical supervision.
Important Warnings
Amitriptyline is highly dangerous in overdose. Even relatively small amounts above the therapeutic dose can cause life-threatening cardiac arrhythmias, seizures, and coma. For this reason, clinicians are cautious about prescribing it to patients considered at risk of self-harm, and quantities dispensed may be limited. If you or someone else has taken too much amitriptyline, call 999 or go to A&E immediately.
Antidepressants including amitriptyline are associated with an increased risk of suicidal thoughts and behaviour in children, adolescents, and young adults (under 25) during the early weeks of treatment. Patients in this age group and their carers should be advised to monitor closely for worsening depression, agitation, or new suicidal thoughts, and to seek immediate help if these occur.
Amitriptyline should be used with extreme caution in patients with a history of heart disease, recent myocardial infarction, heart block, or arrhythmias. Its sodium channel and QT-prolonging effects can worsen existing cardiac conditions. An ECG may be recommended before starting treatment in at-risk patients.
Stopping amitriptyline suddenly can cause a discontinuation syndrome including nausea, headache, flu-like symptoms, insomnia, irritability, and rebound anxiety. Always taper the dose gradually under medical supervision when discontinuing treatment.
