What Is Tacrolimus?
Tacrolimus is a potent calcineurin inhibitor (CNI) immunosuppressant used in two distinct clinical contexts: as a systemic oral or intravenous agent for prevention of organ rejection following solid organ transplantation, and as a topical formulation (0.03% and 0.1% ointment, marketed as Protopic) for the treatment of moderate-to-severe atopic dermatitis (eczema) in patients for whom topical corticosteroids are inappropriate or insufficient.
Systemic tacrolimus (available as immediate-release capsules, modified-release capsules, and granules) is one of the cornerstone immunosuppressants in transplant medicine, with a narrow therapeutic index requiring meticulous plasma level monitoring. Topical tacrolimus ointment works locally on the skin with minimal systemic absorption, offering a steroid-sparing option for eczema management in sensitive areas where prolonged corticosteroid use would be harmful.
Tacrolimus is a prescription-only medication in the UK in all its forms. Both systemic and topical formulations require specialist initiation and careful ongoing clinical oversight.
What Conditions Is Tacrolimus Used For?
Tacrolimus is indicated for:
prevention of acute and chronic rejection following kidney, liver, heart, and other solid organ transplants; used as part of a multi-drug immunosuppressive regimen
topical tacrolimus 0.03% (children aged 2–15 years) and 0.1% (adults) for moderate-to-severe eczema not adequately controlled by topical corticosteroids or where corticosteroid use is contraindicated (e.g., facial, periocular, and intertriginous areas)
used in haematopoietic stem cell transplantation under specialist haematology supervision
Systemic tacrolimus profoundly suppresses the immune system and requires regular blood level monitoring, renal function tests, and clinical review. Topical tacrolimus carries a black-box-level warning in some jurisdictions regarding a theoretical risk of malignancy with long-term use, though current evidence does not establish causation. It is not a first-line treatment for eczema and should be used as directed by a specialist.
How Does Tacrolimus Work?
Tacrolimus belongs to the macrolide family of natural products, originally isolated from the soil bacterium Streptomyces tsukubaensis. Its mechanism of action is centred on inhibition of the calcineurin–NFAT signalling pathway:
Intracellular binding to FKBP-12: Tacrolimus enters cells and binds with high affinity to the cytosolic immunophilin protein FK506-binding protein 12 (FKBP-12). The tacrolimus–FKBP-12 complex then binds to and inhibits calcineurin — a calcium-dependent phosphatase enzyme that is essential for T-cell activation.
Calcineurin inhibition and NFAT blockade: Under normal conditions, calcineurin dephosphorylates the nuclear factor of activated T cells (NFAT), allowing it to translocate into the nucleus and drive transcription of interleukin-2 (IL-2) and other cytokines essential for T-lymphocyte proliferation and activation. By inhibiting calcineurin, tacrolimus prevents NFAT nuclear translocation, blocks IL-2 production, and profoundly suppresses T-cell-mediated immune responses — the principal mechanism of acute organ rejection.
Topical mechanism in eczema: Applied topically to the skin, tacrolimus penetrates the epidermis and inhibits calcineurin in skin-resident T cells and mast cells, suppressing local production of inflammatory cytokines including IL-2, IL-4, IL-5, IFN-γ, and TNF-α. This reduces the cutaneous inflammatory response that drives the itch-scratch cycle of atopic dermatitis without the skin-thinning effects of topical corticosteroids.
Dosages & Administration
The correct dose depends on the type and severity of infection, age, weight, and kidney function. Always follow your clinician's instructions. The table below is for general reference only.
Administration Tips
Systemic tacrolimus: Take on an empty stomach. Food — particularly high-fat meals — significantly reduces absorption. Take at consistent times relative to meals each day.
Do not switch formulations without specialist guidance. Immediate-release and modified-release tacrolimus formulations (e.g., Prograf vs. Advagraf) are not interchangeable without dose recalculation and re-monitoring; formulation switches have been associated with acute rejection events.
Topical tacrolimus: Apply a thin layer only. Use fingertip units to apply a thin smear to affected skin. It is not greasy and can be applied to facial and periocular skin where topical corticosteroids would be harmful.
Minimise sun exposure during topical use. Avoid prolonged sun exposure and use sun protection on areas treated with topical tacrolimus.
Do not use topical tacrolimus under occlusion unless specifically directed.
Side Effects of Tacrolimus
Most side effects are mild and resolve on their own. Serious side effects are rare but require immediate medical attention.
- Nephrotoxicity (reduced renal function)
- Neurotoxicity (tremor, headache, insomnia)
- Hypertension
- Gastrointestinal disturbance (nausea, diarrhoea)
- Serious infections --- systemic immunosuppression markedly increases susceptibility to infections.
- Lymphoma and other malignancies --- prolonged immunosuppression is associated with increased risk.
Systemic tacrolimus has one of the narrowest therapeutic indices of any commonly prescribed medication. Regular trough level measurements are mandatory.
Drug Interactions
Tacrolimus may interact with other medications, including strong CYP3A4 inhibitors and nephrotoxic agents. Always inform your clinician about all medications you are taking.

Important Warnings
Tacrolimus is a powerful immunosuppressant. Transplant patients are at significantly increased risk of opportunistic infections (including CMV, PCP, fungal, and mycobacterial infections) and malignancies, particularly PTLD and non-melanoma skin cancer. Prophylactic antimicrobials are routinely co-prescribed. Annual skin cancer surveillance and rigorous sun protection are essential.
Abrupt discontinuation of tacrolimus in a transplant recipient will precipitate acute rejection, which can be irreversible and lead to graft loss. Any concerns about tolerability must be discussed with the transplant team — never self-discontinue.
Regular trough level measurements are mandatory for all patients on systemic tacrolimus. Patients must attend scheduled monitoring appointments. Any change in concurrent medications must be reported immediately to the transplant team, as many drugs dramatically alter tacrolimus levels.
Tacrolimus impairs insulin secretion; new-onset diabetes mellitus is a well-recognised complication (NODAT), particularly in the first year post-transplant. Blood glucose should be monitored regularly, especially in at-risk patients.

Tacrolimus has an extremely narrow therapeutic window — too little risks organ rejection, too much causes kidney damage and serious toxicity. Blood trough levels must be checked regularly to keep plasma concentrations within the optimal range, and any change in your other medications can dramatically shift your tacrolimus level.
Many common medications — including antifungal creams containing azoles, antibiotics, and even grapefruit juice — significantly alter tacrolimus blood levels. Always consult your transplant pharmacist or clinician before taking any new medication, including supplements and herbal products.
Topical tacrolimus is not recommended as a continuous long-term treatment. It is used intermittently or for defined treatment periods. Systemic absorption from topical use is very low, but prolonged use should be clinician-supervised. Use the minimum amount needed for the shortest time.
Alcohol can cause facial flushing and skin irritation in areas treated with topical tacrolimus. While not dangerous, this can be uncomfortable; many patients choose to avoid alcohol during treatment periods or minimise application before social occasions.
Post-transplant lymphoproliferative disease is a form of lymphoma associated with EBV reactivation in the context of immunosuppression. It is a recognised risk of long-term tacrolimus use. The risk is managed through the lowest effective immunosuppression strategy, regular monitoring, and vigilance for symptoms such as unexplained lymph node swelling, fever, or night sweats — which should always be reported promptly.
