What Is Rosuvastatin (Crestor)?
Rosuvastatin is a synthetic, highly potent HMG-CoA reductase inhibitor — more commonly known as a statin — used to lower elevated blood lipid levels (cholesterol and triglycerides) and to reduce the risk of cardiovascular events including heart attacks, strokes, and cardiovascular death. It is available generically and under the brand name Crestor, manufactured by AstraZeneca. Rosuvastatin is supplied as 5 mg, 10 mg, 20 mg, and 40 mg tablets.
Rosuvastatin is one of the most potent statins available and demonstrates the greatest LDL-cholesterol-lowering efficacy per milligram among the statin class. It is described as a high-intensity statin at doses of 20 mg and 40 mg, meaning it reduces LDL-cholesterol by 50% or more from baseline. Unlike some other statins (particularly simvastatin and atorvastatin), rosuvastatin is not significantly metabolised by the cytochrome P450 3A4 enzyme, giving it a relatively low drug interaction burden. It is predominantly eliminated unchanged or via CYP2C9 metabolism.
What Conditions Does Rosuvastatin Treat?

Rosuvastatin is indicated for the treatment of primary hypercholesterolaemia (high LDL-cholesterol), mixed dyslipidaemia (combined elevation of LDL-cholesterol and triglycerides), and familial hypercholesterolaemia — an inherited disorder causing very high LDL-cholesterol levels from birth that dramatically increases cardiovascular risk. In all these conditions, rosuvastatin significantly reduces total cholesterol, LDL-cholesterol, triglycerides, and apolipoprotein B, while increasing HDL-cholesterol (the "good" cholesterol).
In individuals without established cardiovascular disease but with elevated cardiovascular risk — as determined by risk calculators such as QRISK3, which takes into account age, blood pressure, smoking status, diabetes, family history, and other factors — rosuvastatin is prescribed to reduce the future risk of heart attack, stroke, and cardiovascular death. NICE guidance recommends offering a statin (typically atorvastatin 20 mg first-line, but rosuvastatin at equivalent doses is an alternative) to individuals with a 10-year cardiovascular risk of 10% or greater.
In patients with established atherosclerotic cardiovascular disease (ASCVD) — including prior myocardial infarction, coronary artery disease, peripheral arterial disease, or prior ischaemic stroke — high-intensity statin therapy is standard of care to reduce the risk of further cardiovascular events. The evidence base for intensive LDL-lowering in secondary prevention is very strong; major guidelines (NICE, ESC) recommend an LDL target of <1.4 mmol/L or a >50% reduction from baseline in this group.
Rosuvastatin moderately reduces elevated triglycerides and is useful in mixed dyslipidaemia where both LDL-cholesterol and triglycerides are elevated.
While Rosuvastatin is effective for managing various conditions, it should be used cautiously in patients with renal impairment or those taking other medications that may interact.
How Does Rosuvastatin Work?
HMG-CoA reductase is the rate-limiting enzyme in the intracellular synthesis of cholesterol, catalysing the conversion of HMG-CoA to mevalonate — an early and essential step in the cholesterol biosynthesis pathway. Rosuvastatin is a competitive inhibitor of this enzyme, binding with high affinity and selectively blocking the active site. This reduces intracellular cholesterol synthesis, particularly in hepatocytes (liver cells), which are the primary site of endogenous cholesterol production.
In response to reduced intracellular cholesterol, hepatocytes upregulate the expression of LDL receptors on their surface. These receptors bind and internalise LDL particles from the blood, clearing circulating LDL-cholesterol. The net result is a substantial, dose-dependent reduction in serum LDL-cholesterol — by approximately 45–55% at 10 mg, and up to 60–65% at 40 mg — alongside reductions in total cholesterol, VLDL, triglycerides, and apolipoprotein B, and modest increases in HDL-cholesterol.
Beyond cholesterol lowering, statins including rosuvastatin have pleiotropic effects — additional cardiovascular-protective properties independent of LDL reduction. These include anti-inflammatory effects (reduction in CRP and other inflammatory markers), stabilisation of vulnerable atherosclerotic plaques, improvement in endothelial function, and antithrombotic properties. These pleiotropic effects may contribute to the cardiovascular risk reduction observed with statin therapy beyond what would be predicted from LDL reduction alone.

Administration Tips
Rosuvastatin can be taken at any time of day, with or without food. Unlike some other statins (particularly simvastatin), it does not need to be taken at night. Consistency in timing is more important than the specific time of day. Swallow the tablet whole with water.
Grapefruit and grapefruit juice do not significantly interact with rosuvastatin (unlike simvastatin or atorvastatin, which are metabolised by CYP3A4). This is a clinical advantage.
Antacids containing aluminium and magnesium hydroxide should not be taken at the same time as rosuvastatin — take antacids at least 2 hours after rosuvastatin to avoid reducing absorption.

Drug Interactions

May reduce rosuvastatin absorption if taken simultaneously; administer rosuvastatin at least 2 hours before antacids.
Important Warnings
Report any unexplained muscle pain, tenderness, weakness, or cramps to your clinician, particularly if accompanied by dark or discoloured urine. These may be signs of serious muscle breakdown (rhabdomyolysis), which requires immediate medical assessment.
Do not take rosuvastatin during pregnancy. Use effective contraception while on treatment. Stop immediately and contact your clinician if you become pregnant.
Liver enzymes should be checked before starting rosuvastatin and periodically during treatment. Report any symptoms of liver problems including jaundice, dark urine, or persistent abdominal pain.

All three are statins, but rosuvastatin is more potent milligram-for-milligram than atorvastatin or simvastatin. Rosuvastatin at 20–40 mg achieves LDL reductions of 55–65%, whereas atorvastatin at its maximum dose (80 mg) achieves approximately 50–55%. Rosuvastatin also has a lower risk of drug interactions than simvastatin (which is heavily metabolised by CYP3A4) and can be taken at any time of day unlike simvastatin, which requires evening dosing.
Yes. Liver function tests and lipid levels should be checked before starting treatment. A repeat lipid panel is typically done 3 months after initiation or any dose change to assess response. Routine liver function monitoring during stable long-term therapy is not required in every patient, but is advisable. CK (creatine kinase) should be measured if muscle symptoms develop.
Yes — unlike simvastatin and atorvastatin, rosuvastatin is not significantly metabolised by CYP3A4 and is not affected by grapefruit. This is a practical advantage for patients who consume grapefruit or grapefruit juice.
Cognitive side effects including memory problems and confusion have been reported with statins as a class, but large-scale studies and meta-analyses have not established a causal relationship. In fact, long-term statin use appears to be associated with a reduced risk of dementia in some studies. The FDA in the USA has added a class label warning, but European regulators have not taken the same view. Mild muscle aching without markedly elevated CK is common and often transient. Do not ignore severe muscle pain, weakness, or dark urine — these require urgent medical assessment.
Stopping rosuvastatin will cause LDL-cholesterol levels to return towards their pre-treatment baseline within weeks, as the drug does not permanently alter cholesterol metabolism. For primary and secondary cardiovascular prevention, the ongoing cardiovascular protection depends on continuous treatment. Decisions about stopping should always be made in consultation with your clinician.
