What Is Rivaroxaban (Xarelto)?
Rivaroxaban is an orally administered direct oral anticoagulant (DOAC) — specifically a direct Factor Xa inhibitor — used to prevent and treat thromboembolic conditions including deep vein thrombosis (DVT), pulmonary embolism (PE), and stroke in atrial fibrillation. It is manufactured by Bayer and marketed under the brand name Xarelto. Rivaroxaban is available as 2.5 mg, 10 mg, 15 mg, and 20 mg film-coated tablets.
Rivaroxaban was the first oral direct Factor Xa inhibitor to receive regulatory approval, and along with apixaban, edoxaban, and dabigatran (a direct thrombin inhibitor), belongs to the DOAC class — sometimes called novel oral anticoagulants (NOACs) — that have largely replaced warfarin for many anticoagulation indications. Compared with warfarin, rivaroxaban has a predictable pharmacokinetic profile that does not require routine INR (international normalised ratio) monitoring, has fewer food interactions, and requires no dietary restrictions. However, unlike warfarin, there is less flexibility for dose adjustment without consultation, and its anticoagulant effect cannot be easily monitored with standard coagulation tests.
Andexanet alfa is a specific reversal agent for rivaroxaban (and apixaban), approved for use in the UK in life-threatening or uncontrolled bleeding, providing an antidote not historically available with DOACs.
What Conditions Does Rivaroxaban Treat?
Rivaroxaban is prescribed for:
Atrial fibrillation (AF) is the most common cardiac arrhythmia and causes approximately 30% of all strokes. In AF, the upper chambers of the heart beat irregularly, predisposing to blood clot formation in the left atrial appendage; if these clots embolise to the brain, they cause cardioembolic stroke — typically large and highly disabling. Rivaroxaban 20 mg once daily (15 mg in renal impairment) significantly reduces the risk of stroke and systemic embolism in patients with non-valvular AF (AF not caused by a mechanical heart valve or moderate-to-severe mitral stenosis). The ROCKET-AF trial demonstrated non-inferiority to dose-adjusted warfarin for stroke prevention, with a significantly lower rate of intracranial haemorrhage.
DVT is a blood clot in the deep veins, most commonly in the leg. Rivaroxaban is used to treat acute DVT and to prevent recurrent DVT after the initial treatment phase. The EINSTEIN-DVT and EINSTEIN-Extension trials established rivaroxaban's efficacy and safety in this setting. The treatment regimen uses higher initial doses (15 mg twice daily for 21 days), followed by maintenance (20 mg once daily), mirroring the clinical need for more intense anticoagulation during the acute thrombotic phase.
Pulmonary embolism — blood clot in the pulmonary arteries — is treated with the same rivaroxaban regimen as DVT. Rivaroxaban avoids the need for initial parenteral heparin bridging that is required with warfarin, simplifying management and enabling earlier discharge in suitable low-to-moderate risk patients.
Orthopaedic surgery, particularly hip and knee arthroplasty, carries a high risk of postoperative venous thromboembolism. Rivaroxaban 10 mg once daily is used for thromboprophylaxis: for 5 weeks following total hip replacement and for 2 weeks following total knee replacement.
Rivaroxaban 2.5 mg twice daily, in combination with aspirin 100 mg once daily, is indicated for the secondary prevention of atherothrombotic events in adults with coronary artery disease (CAD) or symptomatic peripheral arterial disease (PAD) at high risk of ischaemic events. The COMPASS trial demonstrated significant reductions in major adverse cardiovascular events (MACE) and major adverse limb events (MALE) with this combination.
While Rivaroxaban is effective for managing various conditions, it should be used cautiously in patients with renal impairment or those taking other medications that may interact.
How Does Rivaroxaban Work?
The coagulation cascade is a sequential series of clotting factor activations that ultimately produces a fibrin clot to stop bleeding. Factor Xa is a pivotal amplification point in this cascade — it acts at the junction of the intrinsic and extrinsic pathways, converting prothrombin to thrombin, which then converts fibrinogen to fibrin. Inhibiting Factor Xa therefore interrupts clot formation upstream of thrombin production.
Rivaroxaban selectively and reversibly inhibits both free Factor Xa (circulating in plasma) and Factor Xa within the prothrombinase complex (where Factor Xa is most active). By blocking Factor Xa, rivaroxaban prevents the conversion of prothrombin to thrombin, inhibiting both clot formation and the amplification of the coagulation cascade. Unlike heparin (which acts indirectly via antithrombin), rivaroxaban acts directly and does not require a cofactor.
The anticoagulant effect is rapid — therapeutic plasma concentrations are achieved within 2 to 4 hours of oral dosing — and the effect is predictable enough that routine monitoring is not required in most patients. The half-life of rivaroxaban is 5 to 13 hours (prolonged in elderly patients due to reduced renal clearance), and the effect wears off within 24 hours in most patients with normal renal function.

Administration Tips
The 15 mg and 20 mg tablets must be taken with food to ensure adequate absorption — bioavailability rises from approximately 66% fasting to near 100% when taken with a meal. The 10 mg and 2.5 mg doses have adequate absorption without food.
For once-daily dosing in AF, taking rivaroxaban with the evening meal is recommended to maximise the proportion of the 24-hour period covered by therapeutic drug levels.
Side Effects of Rivaroxaban
Rivaroxaban is generally well tolerated, but some side effects may occur.
- Easy bruising
- Minor bleeding (e.g. nosebleeds, gum bleeding)
- Anaemia (low red blood cells) — fatigue, weakness
- Nausea
- Major bleeding — prolonged bleeding, blood in urine or stools, coughing or vomiting blood
- Intracranial haemorrhage — severe headache, confusion, weakness, vision changes
- Spinal/epidural haematoma — back pain, numbness, loss of bladder/bowel control (especially after spinal procedures)
- Severe allergic reaction — swelling of face, lips, or throat, difficulty breathing
Patients should be informed about the risk of bleeding and the importance of seeking medical advice if symptoms are not adequately controlled.
Drug Interactions
Inform your clinician about all medications you are taking, as some drugs can interact with Rivaroxaban and affect its efficacy.
(e.g. Warfarin, Heparin, Apixaban) — Additive anticoagulant effects markedly increase bleeding risk. Avoid concurrent use unless specifically indicated and closely supervised.
Important Warnings
Rivaroxaban increases bleeding risk. Seek urgent medical attention for any signs of serious bleeding including: coughing or vomiting blood, blood in urine (red or dark brown), black or tarry stools, severe or unusual bruising, prolonged bleeding from a cut, sudden severe headache, vision changes, or weakness on one side of the body (signs of stroke or intracranial bleed).
Stopping rivaroxaban abruptly without medical guidance significantly increases the risk of stroke (in AF patients) or recurrent DVT/PE. If you need to stop for surgery, follow your clinician's specific advice on when to stop and when to restart.
Inform all healthcare providers, including dentists and surgeons, that you are taking rivaroxaban before any procedure. The appropriate timing for stopping and restarting rivaroxaban depends on the type and invasiveness of the procedure and your indication for anticoagulation.
Unlike warfarin, rivaroxaban does not require routine INR blood tests. However, it does not mean it is without risk. Kidney function should be checked at least annually (as rivaroxaban is partly renally excreted and renal impairment prolongs its effect), and any significant change in medications, kidney function, or health status should prompt a medication review.
While andexanet alfa (Ondexxya) is a licensed reversal agent for rivaroxaban, it is not universally available in all hospitals. Activated charcoal can reduce absorption if taken within 8 hours of a rivaroxaban overdose. In urgent surgical situations, prothrombin complex concentrate (PCC) may be used.

Warfarin's anticoagulant effect varies significantly between patients and over time in the same patient due to genetic variation, diet (especially vitamin K intake), and drug interactions — making regular INR monitoring essential to ensure it remains within the therapeutic range. Rivaroxaban has a predictable, dose-proportional pharmacokinetic profile with minimal food and drug interactions, meaning the anticoagulant effect is consistent enough that routine monitoring is not necessary in most patients. Annual renal function tests are still recommended.
Paracetamol is the preferred painkiller and is safe to use at standard doses with rivaroxaban. NSAIDs such as ibuprofen, naproxen, and diclofenac significantly increase the risk of bleeding — particularly GI bleeding — and should be avoided during rivaroxaban therapy. If you need stronger pain relief, discuss alternatives with your clinician.
For once-daily rivaroxaban (AF or maintenance DVT/PE): take the missed dose as soon as you remember on the same day, then continue the next day as normal. Do not take a double dose on the same day. For twice-daily rivaroxaban (initial DVT/PE treatment): take the missed dose as soon as you remember on the same day. If you only remember at the time of the next dose, skip the missed dose and continue normally — do not double up.
Rivaroxaban is widely used in elderly patients with AF for stroke prevention. Older patients do have a higher bleeding risk, and renal function typically declines with age (increasing rivaroxaban levels). Annual renal function monitoring is particularly important in elderly patients, and dose adjustment may be required. The absolute benefit of anticoagulation for stroke prevention in AF is greater in older patients (who have higher baseline stroke risk), so it remains appropriate for most elderly patients after careful assessment.
Moderate alcohol consumption is generally acceptable, but heavy or binge drinking significantly increases bleeding risk in patients on anticoagulants and should be avoided. Alcohol can also affect renal function and interact with other medications.
Unlike warfarin, rivaroxaban does not interact with vitamin K-containing foods (green leafy vegetables, etc.). There are no dietary restrictions with rivaroxaban, and the dose does not need to be adjusted based on food intake (other than ensuring the 15/20 mg doses are taken with a meal for absorption purposes).
