What sets nebivolol apart from earlier beta-blockers is its additional vasodilatory action. Unlike conventional beta-blockers (which can cause peripheral vasoconstriction — a side effect responsible for cold extremities and worsened symptoms in peripheral arterial disease), nebivolol stimulates the release of nitric oxide (NO) from vascular endothelium, producing direct peripheral vasodilation. This results in a smoother reduction in blood pressure, improved peripheral circulation, and a significantly better tolerability profile — including a substantially lower incidence of sexual dysfunction compared to older beta-blockers.
Its exceptional selectivity for beta-1 receptors (with negligible activity at beta-2 receptors in the lungs) also makes it one of the safest beta-blockers for use in patients with mild-to-moderate obstructive lung disease, though caution is still required.

What Conditions Is Nebivolol Used For?
Nebivolol is indicated for:
long-term treatment of high blood pressure in adults
as an adjunct to standard heart failure therapy in elderly patients (≥70 years) with reduced or preserved ejection fraction
Abrupt withdrawal of beta-blockers — particularly in patients with ischaemic heart disease — can precipitate a severe rebound effect causing angina, myocardial infarction, or dangerous arrhythmias. Always taper nebivolol gradually under medical supervision.
How Does Nebivolol Work?
Nebivolol lowers blood pressure and heart rate through two complementary mechanisms:
Selective beta-1 adrenoceptor blockade in the heart reduces the rate and force of cardiac contraction in response to sympathetic nervous system stimulation. This lowers heart rate (negative chronotropy), reduces myocardial contractility (negative inotropy), and decreases cardiac output — collectively reducing the work the heart must do and lowering blood pressure. Over time, beta-blockade also suppresses renin release from the kidneys, reducing angiotensin II production and further contributing to blood pressure reduction.
Nitric oxide-mediated vasodilation — nebivolol (specifically its D-nebivolol enantiomer) stimulates endothelial nitric oxide synthase (eNOS), increasing the production of nitric oxide in vascular endothelium. NO causes direct relaxation of vascular smooth muscle, producing peripheral vasodilation and lowering peripheral vascular resistance. This vasodilatory component counteracts the reflex peripheral vasoconstriction associated with classic beta-blockers, resulting in better peripheral blood flow and a more favourable haemodynamic profile.
The combination of cardiac rate/output reduction and peripheral vasodilation produces effective, well-tolerated blood pressure control.

- Bronchospasm — wheeze and breathlessness in susceptible patients with asthma or COPD; even highly selective beta-blockers carry some risk
- Severe hypotension — particularly at initiation in volume-depleted patients
- Peripheral ischaemia exacerbation — worsening of symptoms in severe peripheral arterial disease
Do not stop abruptly. Sudden discontinuation of nebivolol — particularly in patients with ischaemic heart disease — can precipitate unstable angina, myocardial infarction, or severe arrhythmia. Always taper gradually over at least 1–2 weeks under medical supervision.
Drug Interactions
Nebivolol may interact with other medications, including MAOIs, CNS depressants, and strong CYP2D6 inhibitors. Always inform your clinician about all medications you are taking.
Additive negative effects on heart rate and AV conduction; risk of severe bradycardia and complete heart block. IV verapamil is absolutely contraindicated with any beta-blocker.
If clonidine is to be discontinued in a patient also on a beta-blocker, the beta-blocker must be withdrawn first and several days allowed before stopping clonidine — to prevent a severe rebound hypertensive crisis.

Nebivolol is contraindicated in patients with bronchial asthma or severe obstructive lung disease. Even highly selective beta-1 blockers retain the potential to provoke bronchospasm in susceptible individuals. Patients with mild-to-moderate COPD who require beta-blockade should be closely monitored and started at very low doses.
Nebivolol must not be used in patients with significant sinus bradycardia (heart rate <60 bpm at rest), greater than first-degree AV block (without a pacemaker), or sick sinus syndrome. Monitor heart rate regularly during treatment.
Beta-blockers blunt the tachycardia that typically alerts diabetic patients to hypoglycaemia. Patients with insulin-dependent diabetes should be warned and should monitor blood glucose more closely.

No — nebivolol is contraindicated in asthma and severe obstructive lung disease. Even the most selective beta-1 blockers can trigger bronchospasm in people with asthma. An alternative antihypertensive class should be used.
Beta-blockers limit the maximum heart rate achievable during exercise. You may notice reduced exercise capacity and a blunted exercise heart rate response. This is a pharmacological effect, not a sign of something wrong, but discuss it with your clinician if it significantly affects your activity levels.
