What Is Lansoprazole?
Lansoprazole is an orally administered proton pump inhibitor (PPI) — a class of drugs that suppress gastric acid secretion by irreversibly inhibiting the hydrogen-potassium ATPase enzyme (proton pump) in the gastric parietal cells. It is one of the most widely prescribed medications in the UK, used for the treatment and prevention of acid-related gastrointestinal conditions. Lansoprazole is available generically and is supplied as 15 mg and 30 mg gastro-resistant capsules. It is also available as dispersible orodispersible tablets for patients with swallowing difficulties.
Lansoprazole is a prodrug — it is absorbed in the small intestine, enters the systemic circulation, and then concentrates in the acidic canaliculi of gastric parietal cells, where it is activated by the acid environment and binds covalently to the proton pump. Because binding is irreversible, acid suppression lasts 24 hours or more per dose, even though the plasma half-life of lansoprazole itself is only 1–2 hours. Recovery of acid secretion requires synthesis of new proton pump proteins, which takes approximately 36–48 hours.
What Conditions Does Lansoprazole Treat?
Lansoprazole is prescribed for:
NSAIDs (ibuprofen, naproxen, diclofenac, aspirin) cause gastric mucosal injury by inhibiting COX-1-mediated prostaglandin synthesis, removing the cytoprotective prostaglandin E2 effect on the gastric lining. Lansoprazole is co-prescribed with NSAIDs in patients at increased risk of peptic ulcer complications (age >65, prior ulcer history, concurrent anticoagulants) to prevent NSAID-associated ulceration and bleeding. It is also used to treat existing NSAID ulcers.
This rare condition involves a gastrin-secreting tumour (gastrinoma) that causes extreme gastric acid hypersecretion, multiple peptic ulcers, and diarrhoea. High-dose PPI therapy (lansoprazole up to 120 mg/day or higher in some cases) controls acid hypersecretion and provides symptomatic relief pending definitive surgical or oncological management.
NICE guidelines recommend a trial of PPI therapy (typically 4 weeks) for uninvestigated dyspepsia in patients under 55 years without alarm features (weight loss, dysphagia, haematemesis, persistent vomiting, or anaemia), while testing and treating for H. pylori is pursued. Lansoprazole 15–30 mg daily is appropriate for this indication.
While Lansoprazole is effective for managing acid-related conditions, it should be used cautiously in patients with a history of hypersensitivity or severe liver impairment.
How Does Lansoprazole Work?
Gastric acid is secreted by parietal cells in the stomach wall via the H+/K+ ATPase enzyme (the proton pump), located on the luminal surface of parietal cell secretory canaliculi. This pump actively transports hydrogen ions (H+) into the gastric lumen in exchange for potassium ions, generating the hydrochloric acid environment of the stomach (pH 1–2).
Lansoprazole is absorbed from the small intestine and transported to gastric parietal cells, where it concentrates in the highly acidic secretory canaliculi. In this acidic environment, lansoprazole is protonated and converted to its active sulfenamide form, which covalently binds to cysteine residues on the H+/K+ ATPase, irreversibly inhibiting it. A single dose of lansoprazole can inhibit approximately 70% of available proton pumps, reducing maximal acid output by up to 80–90%.
Because the inhibition is irreversible, the duration of acid suppression significantly outlasts the plasma drug concentration. Full acid suppression is achieved after 3–5 days of daily dosing, as newly synthesised pumps (which are unbound) are progressively blocked. The consistency of dosing — taking lansoprazole 30–60 minutes before a meal, when the greatest proportion of proton pumps are active — optimises the drug's access to active pumps and maximises its efficacy.

Administration Tips
Lansoprazole gastro-resistant capsules should be swallowed whole — do not crush or chew, as this destroys the gastro-resistant coating and exposes the drug to stomach acid before it reaches the small intestine. They should ideally be taken 30–60 minutes before the first meal of the day, as this is when the greatest number of active proton pumps are available for irreversible inhibition.
For patients who cannot swallow capsules, lansoprazole dispersible (orodispersible) tablets can be dissolved in water and swallowed, or the gastro-resistant granules inside standard capsules can be emptied and mixed with a small amount of acidic soft food (apple sauce, yoghurt) and swallowed immediately without chewing.
Side Effects of Lansoprazole
Lansoprazole is generally well tolerated, but some side effects may occur.
- Headache
- Diarrhoea, nausea, abdominal pain, constipation, flatulence: GI side effects are the most commonly reported; usually mild and transient
- Dizziness
- Hypomagnesaemia (low magnesium): Occurs with prolonged PPI use (typically >1 year); can cause muscle cramps, tetany, arrhythmias, and seizures. Magnesium levels should be checked in patients on long-term PPI therapy, particularly those also on digoxin or diuretics.
- Hyponatraemia (low sodium)
- Clostridium difficile-associated diarrhoea: PPIs reduce the gastric acid barrier, increasing susceptibility to C. difficile infection, particularly in hospitalised or elderly patients. Persistent or severe diarrhoea during PPI therapy should prompt consideration of C. difficile testing.
- Community-acquired pneumonia: Some epidemiological data suggest a modest increased risk, thought to be related to reduced gastric acid defence against aspirated organisms, though causality remains debated.
- Increased fracture risk (hip, wrist, spine): With prolonged high-dose PPI use; mechanism possibly related to reduced calcium absorption in an achlorhydric environment. Risk is small but relevant in long-term use, particularly in patients with osteoporosis risk factors.
- Vitamin B12 deficiency: Reduced gastric acid impairs release of B12 from food proteins; clinically relevant with long-term use (>3 years), particularly in the elderly.
- Acute interstitial nephritis: Rare; immune-mediated renal inflammation that can progress to CKD if not recognised and treated.
- Subacute cutaneous lupus erythematosus (SCLE): Rare cutaneous lupus reaction associated with PPI use; resolves on discontinuation.
Patients should be informed about the risk of hypomagnesaemia and the importance of monitoring magnesium levels during long-term therapy.
Drug Interactions
Inform your clinician about all medications you are taking, as some drugs may interact with Lansoprazole and affect its efficacy.
Lansoprazole may reduce activation of clopidogrel by inhibiting CYP2C19, potentially decreasing antiplatelet effectiveness and increasing cardiovascular risk. Use cautiously and consider alternative PPIs if necessary.
PPIs may reduce methotrexate clearance, increasing risk of methotrexate toxicity. Temporary discontinuation of lansoprazole may be required during high-dose methotrexate therapy.
Reduced gastric acidity may decrease absorption and effectiveness of these medicines. Consider alternative antifungal therapy or adjust administration timing.
Terbinafine may alter warfarin metabolism, increasing anticoagulant effect and bleeding risk. INR should be monitored during and after terbinafine courses.
Important Warnings
Lansoprazole relieves acid-related symptoms but can mask symptoms of gastric cancer or serious oesophageal disease. If you have unexplained weight loss, difficulty swallowing, persistent vomiting, vomiting blood, or black stools, seek urgent medical review — do not simply continue or increase PPI therapy without investigation.
PPIs are among the most overprescribed medications in the UK. Long-term use is associated with hypomagnesaemia, vitamin B12 deficiency, increased fracture risk, and C. difficile susceptibility. Regular clinical review of whether ongoing PPI therapy is still needed, and at the lowest effective dose, is recommended. Do not continue indefinitely without reassessment.
Lansoprazole inhibits the CYP2C19 enzyme needed to activate clopidogrel. In patients taking clopidogrel for cardiovascular protection, lansoprazole may reduce antiplatelet efficacy. Pantoprazole or omeprazole (weaker CYP2C19 inhibitors) are often recommended as alternatives when PPI co-prescription with clopidogrel is necessary.

Some improvement in heartburn and acid reflux symptoms is typically noticed within 2 to 4 days of starting lansoprazole. Full therapeutic effect — particularly for healing oesophagitis or peptic ulcers — develops over 4 to 8 weeks of consistent daily use. Lansoprazole is not a rapid-onset antacid; for immediate symptom relief, an alginate (Gaviscon) or antacid can be taken additionally while waiting for the full PPI effect to establish.
Many patients require long-term PPI therapy for conditions such as chronic GORD, Barrett's oesophagus, or recurrent peptic ulcer disease. When long-term use is indicated, it should be at the lowest effective dose. Annual review of the need for ongoing treatment is recommended. Patients on long-term PPIs should have periodic monitoring of magnesium levels and, in older patients, vitamin B12 levels.
Stopping PPIs abruptly after prolonged use can cause rebound acid hypersecretion — a temporary increase in acid secretion above baseline levels, which can cause a brief worsening of heartburn and reflux symptoms. This is not dangerous but can be uncomfortable and may be misinterpreted as a sign that the PPI is necessary. Gradual step-down (reducing the dose before stopping, or switching to on-demand dosing) is preferable to abrupt discontinuation.
Yes — lansoprazole is specifically used alongside NSAIDs like ibuprofen in patients at high risk of peptic ulceration, to protect the stomach lining. However, this does not mean NSAIDs are risk-free; they can still cause adverse cardiovascular and renal effects. The indication for the NSAID and its duration of use should be regularly reviewed.
Both are proton pump inhibitors with broadly equivalent efficacy for acid suppression. Lansoprazole 30 mg is approximately equivalent to omeprazole 20 mg. Practical differences include that lansoprazole is less inhibitory of CYP2C19 than omeprazole for some purposes, though both can reduce clopidogrel efficacy. Omeprazole has a larger dataset for use in pregnancy. Choice between them is largely based on local prescribing guidelines, cost, and individual factors.
