Famotidine is available as 20 mg and 40 mg tablets. It is rapidly acting (onset within 1 hour) and has a duration of action of approximately 10 to 12 hours, making it suitable for twice-daily dosing or evening dosing for nocturnal acid suppression.

What Conditions Is Famotidine Used For?
Famotidine is prescribed for:
relief of heartburn, acid regurgitation, and oesophagitis; particularly useful for nocturnal acid breakthrough in patients on PPIs.

How Does Famotidine Work?
Famotidine competitively and reversibly blocks histamine H2 receptors on the basolateral membrane of gastric parietal cells. Histamine — released by enterochromaffin-like (ECL) cells of the gastric mucosa in response to gastrin and vagal stimulation — is the primary stimulant of acid secretion, acting via H2 receptors to activate adenylyl cyclase and increase intracellular cAMP, ultimately activating the proton pump (H⁺/K⁺-ATPase).
By blocking H2 receptors, famotidine reduces both basal (fasting) and meal-stimulated gastric acid secretion, as well as nocturnal acid production. Unlike PPIs, which irreversibly inhibit the proton pump, famotidine's H2 blockade is competitive and reversible — its effects are somewhat shorter-lived (approximately 10 to 12 hours), meaning twice-daily dosing is often needed for sustained 24-hour acid suppression.
Tolerance (tachyphylaxis) can develop with regular H2 blocker use — sustained acid suppression is somewhat diminished after continuous daily use for 1 to 2 weeks, which limits their utility for long-term maintenance compared to PPIs. However, famotidine retains its effectiveness for on-demand or nocturnal use.

Administration Tips
For nocturnal symptoms or ulcer healing, taking the full daily dose at night is often preferred — acid secretion is highest at night, and a single bedtime dose targets this peak effectively.
For twice-daily dosing, take one dose in the morning and one at bedtime.
Famotidine works relatively quickly (within 1 hour) compared to PPIs, making it suitable for on-demand use before a meal known to trigger symptoms.
Avoid taking antacids within 1 hour of famotidine, as antacids may impair its absorption.

Famotidine is primarily renally excreted. In patients with significant renal impairment, drug accumulation can cause CNS side effects including confusion and hallucinations. Dose reduction or extended dosing intervals are required. Inform your clinician of any renal impairment before starting famotidine.
Drug Interactions
Famotidine has a significantly cleaner drug interaction profile than cimetidine, which is a potent CYP450 inhibitor. The following interactions apply:
Reduced gastric acidity impairs the absorption of pH-dependent drugs.
May reduce famotidine absorption if taken simultaneously; space doses by at least 2 hours.
Reduces renal excretion of famotidine; modest increase in famotidine levels.
May modestly reduce famotidine absorption; separate by at least 1 hour.
Important Warnings
Unlike PPIs, H2 blockers including famotidine develop tolerance (tachyphylaxis) with continuous daily use — typically after 1 to 2 weeks of regular dosing. For long-term acid suppression, PPIs are generally more effective. Famotidine is best used on an as-needed basis, for short courses, or as an add-on to PPI therapy for nocturnal symptoms.
As with all acid-suppressing medications, long-term use without investigation risks masking symptoms of gastric cancer. Alarm symptoms (unexplained weight loss, dysphagia, anaemia, vomiting) require endoscopic investigation before or despite acid suppression.
Famotidine is considered one of the safer options for managing acid reflux during pregnancy and has been used extensively in this context. It crosses the placenta and is detectable in breast milk in small amounts. Discuss with your clinician before use in pregnancy or breastfeeding.
