How Does Diclofenac Potassium Work?
Diclofenac is a non-selective cyclooxygenase (COX) inhibitor, meaning it inhibits both the COX-1 and COX-2 isoforms of the cyclooxygenase enzyme, though it has some preferential activity towards COX-2 compared to many other traditional NSAIDs.
The cyclooxygenase enzymes are responsible for converting arachidonic acid -- a fatty acid released from cell membranes in response to tissue damage or irritation -- into prostaglandins and thromboxanes. Prostaglandins are the key chemical mediators of inflammation: they sensitise pain receptors (nociceptors), cause vasodilation and increased vascular permeability (leading to swelling and redness), contribute to fever by acting on the hypothalamus, and stimulate uterine contractions (causing menstrual pain). By blocking COX enzymes, diclofenac reduces prostaglandin synthesis throughout the body, producing its analgesic, anti-inflammatory, and antipyretic effects.
COX-1 is constitutively expressed (always present) in many tissues and plays important protective roles -- including maintaining the gastric mucosal lining by stimulating protective prostaglandins, supporting platelet aggregation via thromboxane A2, and regulating renal blood flow. Inhibition of COX-1 is responsible for the gastrointestinal side effects, platelet inhibition, and renal effects associated with NSAID use.

Administration Tips
Take diclofenac potassium with food or milk, or immediately after eating. Despite its rapid-release formulation, food helps protect the stomach lining and reduces the risk of gastrointestinal irritation, nausea, and dyspepsia. If taking it for migraine -- where speed of absorption is paramount -- it can be taken with a small amount of food if tolerated.
Take with a full glass of water and remain upright for at least 30 minutes after taking to reduce the risk of oesophageal irritation.
Use the lowest effective dose for the shortest possible time. NSAIDs including diclofenac carry dose-dependent risks for gastrointestinal, cardiovascular, and renal complications. The principle of minimum necessary dose and duration applies particularly strongly to diclofenac given its cardiovascular risk profile.
Do not take with other NSAIDs -- including over-the-counter ibuprofen or aspirin (at anti-inflammatory doses) -- as combining NSAIDs increases the risk of gastrointestinal bleeding without improving pain relief.

Side Effects of Diclofenac Potassium
Diclofenac is generally well tolerated for short-term use in appropriate patients. The risk of both gastrointestinal and cardiovascular side effects increases with dose, duration of use, and in the presence of pre-existing risk factors.

Approximately 10% of adults with asthma and a higher proportion of those with nasal polyps have aspirin- or NSAID-exacerbated respiratory disease (AERD), in which NSAIDs including diclofenac trigger bronchospasm and severe asthma attacks. Patients with asthma should use diclofenac with caution and must stop immediately and seek emergency care if they experience any worsening of breathing after taking it. A history of aspirin- or NSAID-triggered wheeze is a contraindication.
Drug Interactions
Diclofenac has a number of clinically important drug interactions. Always disclose all prescription drugs, over-the-counter medicines, vitamins, and supplements to your clinician or pharmacist before starting diclofenac.
Diclofenac impairs platelet function and can cause gastrointestinal bleeding, while anticoagulants prevent clotting. This combination significantly increases the risk of serious and potentially fatal bleeding. The combination should generally be avoided; if unavoidable, it requires close clinical monitoring and gastroprotection.
Combining diclofenac with other NSAIDs provides no additional analgesic benefit and substantially increases the risk of gastrointestinal bleeding and adverse effects. This combination is contraindicated. Low-dose aspirin (75 mg) for cardiovascular protection may be continued but also increases GI bleeding risk.
NSAIDs antagonise the blood pressure-lowering effects of antihypertensives and diuretics, and the combination can cause acute kidney injury -- particularly in elderly or dehydrated patients (the "triple whammy" interaction: ACE inhibitor + diuretic + NSAID). Blood pressure and renal function should be monitored.
Diclofenac reduces the renal clearance of lithium, causing blood levels to rise and increasing the risk of lithium toxicity. Lithium levels should be monitored closely when starting or stopping diclofenac.
Important Warnings
Diclofenac has a higher cardiovascular risk profile than most other NSAIDs, including ibuprofen and naproxen. It is associated with an increased risk of myocardial infarction, stroke, and heart failure, particularly with prolonged use or at higher doses. This risk is comparable to that of selective COX-2 inhibitors (coxibs). Diclofenac is contraindicated in patients with established ischaemic heart disease, peripheral arterial disease, cerebrovascular disease, or heart failure. In patients with cardiovascular risk factors, the decision to prescribe must involve a careful assessment of individual risk and benefit.
Bleeding and Perforation All NSAIDs, including diclofenac, carry a risk of serious gastrointestinal events including ulceration, bleeding, and perforation of the stomach or intestine. These events can occur without warning symptoms at any point during treatment -- not only at the start. Elderly patients, those with a history of peptic ulcer disease, those on anticoagulants or corticosteroids, and those who smoke or consume alcohol regularly are at highest risk. Gastroprotection with a PPI should be co-prescribed in at-risk patients.
Diclofenac has a higher rate of liver enzyme elevation than many other NSAIDs. Clinically significant hepatotoxicity, including severe hepatitis and liver failure, has been reported, albeit rarely. Liver function should be monitored in patients on prolonged courses, and diclofenac should be avoided in patients with significant hepatic impairment. Symptoms of liver toxicity -- jaundice, dark urine, right upper abdominal pain -- require immediate cessation of treatment and medical review.
Diclofenac is contraindicated in patients with a history of bronchospasm, urticaria, or rhinitis triggered by aspirin or other NSAIDs. In asthmatic patients without known NSAID sensitivity, diclofenac should be used with caution. Worsening of asthma during NSAID treatment requires immediate cessation and medical review.
Diclofenac is contraindicated from 20 weeks of pregnancy onwards due to the risk of foetal renal dysfunction, oligohydramnios (reduced amniotic fluid), and premature closure of the ductus arteriosus (a critical foetal blood vessel), which can cause serious and potentially fatal complications. It should be used with caution and only if absolutely necessary in the first and second trimesters. Diclofenac passes into breast milk in small amounts; short-term use at low doses is generally considered acceptable during breastfeeding but should be discussed with your clinician.
