What Is Colchicine?

Colchicine is one of the oldest medicines still in active clinical use, derived from the autumn crocus plant (Colchicum autumnale) and used medicinally for over two thousand years. Despite its ancient origins, it remains a modern, evidence-based treatment and is the first-line option for the treatment and prevention of acute gout attacks, as well as a key treatment for familial Mediterranean fever and pericarditis.

Colchicine works by a unique mechanism -- it does not reduce uric acid levels (the underlying cause of gout) and is not an anti-inflammatory drug in the conventional sense. Instead, it specifically disrupts the inflammatory cascade triggered by uric acid crystals in the joints, preventing the acute inflammatory response from becoming established. This highly targeted action makes it remarkably effective for gout when used correctly, but also means it must be used at the right time and in the right dose to achieve the best results.

At the doses now recommended in modern clinical practice, colchicine is considerably safer than older high-dose regimens, with a much more acceptable side effect profile. The key to its safe use is understanding that the therapeutic and toxic doses are relatively close together, making correct dosing critically important.

Colchicine is available as tablets and is suitable for adults. It is available generically and under the brand name Colcrys.

What It Treats

What Conditions Is Colchicine Used For?

Colchicine is used for several distinct conditions, each exploiting different aspects of its anti-inflammatory mechanism:

Acute Gout

the primary indication; highly effective at relieving the intense pain, swelling, redness, and heat of an acute gout attack when started early; most effective when taken within the first 12--24 hours of symptom onset.

Gout Prophylaxis

low-dose colchicine taken daily is used to prevent acute gout attacks when starting urate-lowering therapy (such as allopurinol), as changes in uric acid levels can paradoxically trigger attacks during the initiation phase.

Familial Mediterranean Fever (FMF)

a rare inherited autoinflammatory condition causing recurrent episodes of fever, serositis, and joint pain; long-term daily colchicine is the standard of care and dramatically reduces the frequency and severity of attacks, and prevents the development of AA amyloidosis.

Acute Pericarditis

inflammation of the pericardium (the sac surrounding the heart); colchicine combined with NSAIDs is the recommended first-line treatment, reducing the risk of recurrence by approximately 50%.

Recurrent Pericarditis

long-term colchicine is used to prevent further episodes in patients with recurrent pericarditis.

Pericardial Effusion

colchicine may be used as part of management to reduce inflammation and prevent recurrence.

Colchicine Does Not Lower Uric Acid

Colchicine treats the inflammation of a gout attack and prevents attacks from occurring during urate-lowering therapy -- but it does not reduce the underlying serum uric acid levels that cause gout. Long-term management of gout also requires urate-lowering therapy (typically allopurinol or febuxostat) to reduce uric acid production and prevent future attacks and joint damage. Mechanism of Action

How Does Colchicine Work?

Colchicine's mechanism of action is distinct from all other anti-inflammatory medications and operates primarily at the level of cellular microtubule dynamics and neutrophil function.

Microtubule disruption: Colchicine binds with high affinity to tubulin -- the protein subunit from which microtubules are assembled. Microtubules are essential cytoskeletal structures involved in cell division, intracellular transport, and the mobility and activation of immune cells. By binding to tubulin and preventing its polymerisation into microtubules, colchicine disrupts the normal functioning of cells that are highly dependent on microtubule dynamics -- particularly neutrophils (the white blood cells primarily responsible for the acute inflammatory response in gout).

Neutrophil inhibition: In gout, uric acid crystals deposit in joints and are phagocytosed (engulfed) by neutrophils, triggering the release of inflammatory mediators including interleukin-1β (IL-1β), which drives the intense pain, swelling, and redness of an acute attack. Colchicine inhibits multiple steps in this process: it impairs neutrophil migration to the site of inflammation (chemotaxis), reduces neutrophil adhesion to endothelial cells, and inhibits the assembly of the NLRP3 inflammasome -- the key molecular platform responsible for activating IL-1β. By blocking these upstream steps, colchicine effectively prevents the inflammatory cascade from becoming fully established.

Crucially, colchicine does not block cyclooxygenase enzymes (as NSAIDs do), does not inhibit prostaglandin synthesis, and has no direct analgesic properties. Its anti-inflammatory effects are entirely mediated through neutrophil and microtubule biology, which is why it is so specifically effective for crystal-induced arthritis and conditions driven by neutrophil-mediated inflammation, while having relatively little effect on other types of inflammatory conditions.

Administration Tips

Start colchicine as early as possible during a gout attack. Colchicine is most effective when started within 12--24 hours of the onset of symptoms. Starting it late in an established attack significantly reduces its effectiveness. If colchicine is prescribed and kept at home for use at the first sign of an attack, ensure you know the correct dose and when to start it.

Do not exceed the prescribed dose. The difference between a therapeutic and a toxic dose of colchicine is relatively narrow. The old practice of taking colchicine until symptoms resolved or diarrhoea occurred is no longer recommended. Modern low-dose regimens achieve equivalent efficacy with significantly fewer side effects.

Colchicine can be taken with or without food. If gastrointestinal side effects occur, taking it with food may help.

Avoid grapefruit juice during colchicine treatment. Grapefruit inhibits CYP3A4, the enzyme responsible for metabolising colchicine, causing blood levels to rise significantly.

Do not take colchicine with certain other medications without medical guidance -- particularly strong CYP3A4 or P-glycoprotein inhibitors (see interactions below), which can cause colchicine to accumulate to toxic levels.

Side Effects of Colchicine

At the low doses used in modern clinical practice, colchicine is generally well tolerated. Gastrointestinal side effects are the most common. Serious toxicity is rare at therapeutic doses but can occur -- particularly when interacting medications are co-administered or in patients with renal or hepatic impairment.

  • Colchicine toxicity / overdose -- multisystem organ failure can occur with significant overdose; presents initially with severe gastrointestinal symptoms followed by bone marrow suppression, cardiovascular collapse, and neuromuscular failure; requires immediate emergency care
  • Severe myelosuppression -- anaemia, leucopenia, thrombocytopenia; more likely with prolonged use or drug interactions; presents as unusual fatigue, bruising, or recurrent infections
  • Rhabdomyolysis -- severe muscle breakdown particularly when combined with statins; presents as muscle pain, weakness, and dark urine
  • Severe allergic reaction -- rare; urticaria, swelling, difficulty breathing
Colchicine Has a Narrow Therapeutic Window

The difference between an effective and a toxic dose of colchicine is smaller than with many other medications. This is particularly relevant when other drugs that inhibit its metabolism are co-administered -- such as clarithromycin, ciclosporin, or strong CYP3A4 inhibitors -- which can cause colchicine to accumulate to dangerous levels even at standard doses. Always inform your clinician and pharmacist of all medications you are taking before starting colchicine.

Drug Interactions

Colchicine is metabolised by CYP3A4 and is a substrate of the P-glycoprotein (P-gp) drug transporter. Inhibitors of either pathway can significantly increase colchicine blood levels and the risk of serious toxicity. Always disclose all medications to your clinician or pharmacist.

Major
Strong CYP3A4 and P-gp Inhibitors (e.g. Clarithromycin, Erythromycin, Ciclosporin, Itraconazole, Ketoconazole, Ritonavir)

These drugs dramatically increase colchicine blood levels, potentially causing life-threatening colchicine toxicity including myelosuppression and multi-organ failure -- even when colchicine is taken at a standard therapeutic dose. Concurrent use is contraindicated in patients with renal or hepatic impairment and requires significant dose reduction with close monitoring in others.

Major
Statins (e.g. Atorvastatin, Simvastatin)

Combining colchicine with statins increases the risk of myopathy and rhabdomyolysis. Both drugs can independently cause muscle toxicity and the combination amplifies this risk. Monitor for muscle pain, weakness, and dark urine. Temporary suspension of the statin during short colchicine courses may be considered.

Moderate
Digoxin

Colchicine may increase digoxin blood levels through P-gp inhibition. Digoxin levels should be monitored during concurrent use.

Minor
Vitamin B12

Long-term colchicine use can impair the absorption of vitamin B12 in the small intestine. B12 levels should be monitored in patients on long-term colchicine therapy.

Colchicine Toxicity

Colchicine poisoning is a medical emergency. Even relatively modest doses in excess of the therapeutic range -- or standard doses combined with interacting medications -- can cause life-threatening multisystem toxicity. Early symptoms include severe nausea, vomiting, and diarrhoea, followed by bone marrow failure, cardiovascular collapse, neuromuscular failure, and respiratory depression. There is no specific antidote. If colchicine toxicity is suspected, call 999 immediately.

Interactions with Macrolide Antibiotics

The interaction between colchicine and macrolide antibiotics -- particularly clarithromycin and erythromycin -- is one of the most dangerous drug interactions in clinical practice. These antibiotics are potent CYP3A4 and P-gp inhibitors and can cause colchicine to accumulate to life-threatening levels even when taken at a standard therapeutic dose. Deaths have been reported from this interaction. Patients taking colchicine who are prescribed a macrolide antibiotic must inform their clinician immediately. Azithromycin is a safer alternative macrolide with less CYP3A4 inhibition.

Renal and Hepatic Impairment

Colchicine is excreted primarily by the kidneys and metabolised by the liver. Patients with significant renal or hepatic impairment are at high risk of colchicine accumulation and toxicity. Dose reduction is required in moderate impairment, and colchicine is contraindicated in severe renal or hepatic failure. Kidney function should be assessed before starting long-term colchicine therapy.

Elderly patients are at increased risk of colchicine toxicity due to age-related decline in renal and hepatic function, reduced drug clearance, lower muscle mass, and a higher likelihood of co-prescribed interacting medications. Lower doses and careful monitoring are required in this population.

Can I take colchicine with ibuprofen?

For acute gout, both NSAIDs (such as ibuprofen or naproxen) and colchicine are effective treatments, but they are generally used as alternatives rather than together, as combining them does not significantly improve outcomes and increases the risk of gastrointestinal side effects. For pericarditis, colchicine is specifically recommended in combination with NSAIDs as this combination has been shown to halve the risk of recurrence. Your clinician will advise on the most appropriate regimen for your specific condition.

I have been prescribed clarithromycin -- can I still take colchicine?

This is an urgent safety concern. Clarithromycin is a potent inhibitor of the enzymes that metabolise colchicine, and combining the two can cause colchicine to accumulate to life-threatening levels -- even at a standard therapeutic dose. Deaths have been reported from this combination. You must inform your prescribing clinician immediately that you take colchicine. An alternative antibiotic (such as azithromycin, which has less interaction potential) may be more appropriate.

How is colchicine different from NSAIDs for treating gout?

Both colchicine and NSAIDs are effective for acute gout, but they work through entirely different mechanisms. NSAIDs (such as ibuprofen or naproxen) reduce inflammation by inhibiting cyclooxygenase enzymes and prostaglandin synthesis. Colchicine works specifically by disrupting neutrophil function and the NLRP3 inflammasome pathway -- the specific inflammatory cascade activated by uric acid crystals. NSAIDs are generally preferred when prompt, potent anti-inflammatory effect is needed and there are no contraindications. Colchicine is preferred when NSAIDs are contraindicated (e.g. in patients with peptic ulcer disease, renal impairment, or on anticoagulants), and for prophylaxis during urate-lowering therapy initiation.

Is colchicine safe to take long-term?

Low-dose colchicine (500 mcg once or twice daily) is considered safe for long-term use in most patients and has been used for decades in conditions such as familial Mediterranean fever. Long-term use requires periodic monitoring of blood counts and kidney function, and awareness of the cumulative risk of peripheral neuropathy and myopathy. Patients on long-term colchicine should have their medication reviewed regularly and vitamin B12 levels monitored, as colchicine can impair B12 absorption.
