What Is Clopidogrel?
Clopidogrel is an orally administered antiplatelet agent belonging to the thienopyridine class. It is used to reduce the risk of atherothrombotic events — heart attacks, strokes, and cardiovascular death — in patients with established atherosclerotic cardiovascular disease, and in combination with aspirin following acute coronary syndromes or coronary stent placement. Clopidogrel is available generically and previously under the brand name Plavix. It is supplied as 75 mg tablets for maintenance therapy and 300 mg tablets for loading doses in acute settings.
Clopidogrel is a prodrug that requires hepatic conversion by cytochrome P450 enzymes (predominantly CYP2C19) to its active thiol metabolite. This metabolite irreversibly inhibits the P2Y12 receptor on platelets, blocking ADP-induced platelet activation and aggregation. Because the inhibition is irreversible, platelet function is impaired for the lifetime of the platelet — approximately 7–10 days — meaning that the antiplatelet effect of a single dose persists until sufficient new (uninhibited) platelets are produced to restore normal haemostasis.

Clopidogrel 75 mg daily is recommended as first-line long-term secondary prevention following ischaemic stroke or transient ischaemic attack (TIA). The CAPRIE trial demonstrated that clopidogrel was modestly but statistically significantly superior to aspirin in reducing the composite outcome of ischaemic stroke, MI, and vascular death in patients with recent stroke, MI, or peripheral arterial disease. NICE guidance recommends clopidogrel monotherapy (rather than aspirin) as the preferred long-term antiplatelet agent for secondary stroke prevention.
Following TIA or minor ischaemic stroke, short-term DAPT (clopidogrel + aspirin for 21–30 days) is now recommended by NICE and supported by the POINT and CHANCE trial data, to reduce the high early risk of recurrent stroke before reverting to clopidogrel monotherapy.
Clopidogrel reduces the risk of ischaemic events (MI, stroke, limb ischaemia) in patients with symptomatic peripheral arterial disease (claudication, critical limb ischaemia, prior peripheral arterial revascularisation). The CAPRIE trial supported this indication.
Clopidogrel may be used as an alternative to aspirin for long-term secondary prevention of cardiovascular events in patients with stable coronary artery disease who are aspirin-intolerant.
While Clopidogrel is effective for reducing the risk of heart attacks and strokes, it should be used cautiously in patients with a history of bleeding disorders or hypersensitivity to the drug.
How Does Clopidogrel Work?
Platelets play a central role in the pathophysiology of arterial thrombosis — the occlusive clots that cause myocardial infarction and ischaemic stroke. When an atherosclerotic plaque ruptures or arterial endothelium is damaged, platelets adhere to the exposed subendothelial matrix and are activated by multiple stimuli, including adenosine diphosphate (ADP) released from activated platelets and damaged cells.
ADP activates the P2Y12 receptor on the platelet surface, amplifying platelet activation, promoting platelet aggregation (clumping together), and reinforcing the haemostatic plug that can occlude arteries. The P2Y12 receptor also suppresses the intracellular cAMP signalling that normally limits platelet activation.
Clopidogrel is a prodrug. After oral absorption, it undergoes two-step hepatic oxidation, predominantly via CYP2C19 (and to a lesser extent CYP3A4 and CYP2C9), to produce an active thiol metabolite. This metabolite irreversibly binds to and blocks the P2Y12 receptor via a disulphide bond, permanently inactivating that receptor. Because the blockade is irreversible, each platelet remains inhibited for its entire lifespan (approximately 7–10 days). Normal platelet function gradually recovers as old inhibited platelets are cleared from circulation and newly produced uninhibited platelets replace them.
The net effect is reduced platelet aggregation, decreased thrombus formation at sites of arterial injury, and reduced risk of atherothrombotic events.

Administration Tips
Clopidogrel 75 mg tablets can be taken at any time of day, with or without food. Once-daily dosing at the same time each day improves adherence. Tablets should be swallowed whole with water. Loading doses (300–600 mg) are administered in acute hospital settings under medical supervision.b
Side Effects of Clopidogrel
Clopidogrel is generally well tolerated, but some side effects may occur.
- Minor bruising
- Nausea
- Abdominal pain
- Thrombotic thrombocytopenic purpura (TTP) — a rare but serious condition.
Patients should be informed about the risk of bleeding and the importance of not stopping Clopidogrel without medical advice.
Drug Interactions
Clopidogrel may interact with other medications, including anticoagulants and NSAIDs, increasing the risk of bleeding. Always inform your clinician about all medications you are taking.
These medicines inhibit CYP2C19, reducing clopidogrel activation and decreasing antiplatelet effectiveness, increasing risk of cardiovascular events. Avoid omeprazole/esomeprazole where possible; consider alternatives such as pantoprazole.
May reduce conversion of clopidogrel to its active metabolite, decreasing antiplatelet effectiveness and increasing thrombotic risk. Avoid concurrent use where possible.
Additive gastrointestinal irritation and antiplatelet effects increase the risk of gastrointestinal bleeding. Use cautiously and consider gastroprotection.
May increase bleeding risk due to effects on platelet function. Monitor for signs of bleeding, especially during long-term therapy.
Important Warnings
Clopidogrel — particularly when used as part of DAPT after coronary stent placement — must not be stopped abruptly without urgent medical consultation. Stopping antiplatelet therapy prematurely after a drug-eluting stent is one of the most common triggers of catastrophic in-stent thrombosis and acute MI. If surgery or a procedure is planned, the surgical and cardiology teams must discuss the timing of clopidogrel interruption.
Inform all clinicians, dentists, and surgeons that you are taking clopidogrel before any procedure. Bleeding times are significantly prolonged. Many dental and minor surgical procedures can proceed on clopidogrel, but the prescribing team must be consulted for higher-risk interventions.
Seek urgent medical attention if you develop unusual bruising or bleeding, dark urine, jaundice, extreme fatigue, confusion, or fever while taking clopidogrel — these may be symptoms of TTP, a rare but potentially fatal complication requiring immediate specialist management.
Seek medical attention for any signs of unusual or severe bleeding including black or tarry stools, vomiting blood, coughing up blood, prolonged bleeding from a cut, severe or unusual bruising, or blood in urine.

Stopping without medical advice dramatically increases the risk of a further heart attack, stroke, or, if you have a coronary stent, potentially fatal in-stent thrombosis. Always discuss any planned changes to clopidogrel with your prescribing clinician before stopping.
After a heart attack or coronary stent placement, the combination of two antiplatelet drugs (clopidogrel + aspirin, known as DAPT) is more effective at preventing stent thrombosis and recurrent coronary events during the vulnerable early healing period than either drug alone. However, DAPT increases bleeding risk compared with single antiplatelet therapy, so its duration is carefully determined based on your specific clinical situation. A proportion of the population — CYP2C19 poor metabolisers — carry genetic variants that reduce this enzyme's activity, resulting in lower levels of the active metabolite and reduced platelet inhibition. These individuals may not receive the full benefit of clopidogrel and may be at higher risk of events after ACS or stenting. In high-risk situations, your clinician may consider testing for this genetic variant or switching to an alternative antiplatelet drug (such as ticagrelor) that does not require metabolic activation.
Yes. Regular ibuprofen and other NSAIDs significantly increase the risk of GI bleeding in patients on antiplatelet therapy. Paracetamol at standard doses is a safer alternative for pain relief. If anti-inflammatory treatment is genuinely necessary, discuss the appropriate option with your clinician.
This depends on why it was prescribed. After ischaemic stroke or TIA, clopidogrel is typically a lifelong medication. After an acute coronary syndrome or coronary stent placement, the duration of DAPT (clopidogrel + aspirin) is determined by stent type, bleeding risk, and clinical context — typically 6–12 months — after which the ongoing antiplatelet regimen is reviewed. Never stop or change clopidogrel without prior discussion with your prescribing clinician or specialist.
