What Is Bisoprolol?
Bisoprolol fumarate is a highly selective beta-1 adrenergic receptor blocker (beta-blocker) used in the management of hypertension, heart failure, angina pectoris, and a range of cardiac arrhythmias, most notably atrial fibrillation (AF) for rate control. It is available in tablets of 1.25 mg, 2.5 mg, 3.75 mg, 5 mg, 7.5 mg, and 10 mg.
Bisoprolol's high cardioselectivity — its preferential action on cardiac beta-1 receptors rather than pulmonary beta-2 receptors — means it has a more favourable profile for patients with mild-to-moderate reactive airway disease compared to older, non-selective beta-blockers. However, it is not entirely without beta-2 effects at higher doses, and must still be used with caution in asthma.
Bisoprolol is a prescription-only medication in the UK. A clinician must assess your cardiac status, respiratory history, blood pressure, and resting heart rate before prescribing.
What Conditions Is Bisoprolol Used For?

In heart failure, bisoprolol must be started at a very low dose (1.25 mg) and up-titrated slowly over weeks to months under clinical supervision. Initiating at too high a dose or escalating too rapidly can precipitate acute decompensation. Never stop bisoprolol suddenly — this can cause rebound tachycardia, angina, and in susceptible patients, myocardial infarction.
How Does Bisoprolol Work?
Bisoprolol is a highly beta-1 selective competitive antagonist at adrenergic receptors. Its mechanism of action differs depending on the clinical target:
In hypertension: Bisoprolol reduces cardiac output by slowing heart rate (negative chronotropy) and reducing myocardial contractility (negative inotropy). Over time, it also suppresses renin release from the juxtaglomerular apparatus of the kidney, attenuating the renin-angiotensin-aldosterone system and reducing systemic vascular resistance.
In angina: By reducing resting and exercise-induced heart rate and myocardial contractility, bisoprolol substantially lowers myocardial oxygen demand — allowing ischaemic myocardium to remain perfused without triggering anginal pain at lower levels of exertion.
In heart failure: Chronic sympathetic nervous system overactivation is a principal driver of progressive cardiac remodelling in heart failure. Bisoprolol antagonises this harmful catecholamine excess at cardiac beta-1 receptors, reversing pathological remodelling, improving ejection fraction over time, and reducing the risk of fatal arrhythmias.
In atrial fibrillation: Bisoprolol slows conduction through the atrioventricular (AV) node, reducing the ventricular rate in AF and improving diastolic filling time, cardiac output, and symptoms.
Dosages & Administration

Administration Tips
Take bisoprolol at the same time each day, preferably in the morning to align with physiological heart rate patterns.
Can be taken with or without food. Food does not significantly affect absorption.
Never stop bisoprolol suddenly. Abrupt withdrawal causes rebound sympathetic activation — with rapid heart rate, worsening angina, and in patients with ischaemic heart disease, potential myocardial infarction. Doses must be reduced gradually over 1–2 weeks under medical supervision.
Monitor your resting heart rate. Bisoprolol should not be started or continued if your resting heart rate is below 50–55 beats per minute (bpm). Report bradycardia symptoms (dizziness, fainting, extreme fatigue) promptly.
In heart failure, titrate slowly. Dose increases should occur no more often than every 2 weeks, and each step should be clinically reviewed.
Side Effects of Bisoprolol
Most side effects are mild and resolve on their own. Serious side effects are rare but require immediate medical attention.
- Fatigue and reduced exercise tolerance (very common, particularly at initiation)
- Cold extremities (hands and feet)
- Bradycardia (slow heart rate)
- Dizziness, particularly on standing (orthostatic hypotension)
- Headache
- Sleep disturbance (vivid dreams or insomnia) — less common with bisoprolol than with less selective beta-blockers
- Nausea or gastrointestinal discomfort
- Severe bradycardia or heart block --- heart rate
- Acute heart failure decompensation --- particularly if bisoprolol is started or up-titrated too quickly in heart failure patients; increasing breathlessness, oedema, or orthopnoea
- Bronchospasm --- even at therapeutic doses, bisoprolol can trigger airway narrowing in patients with asthma or severe COPD; seek immediate care if chest tightness or wheeze develops
- Masking of hypoglycaemia --- beta-blockers blunt the tachycardia that warns diabetic patients of hypoglycaemia; sweating is preserved but other warning signs are masked
- Raynaud's phenomenon exacerbation --- severe peripheral vasospasm in susceptible patients
Although bisoprolol is more cardioselective than older beta-blockers, it is not completely safe in asthma. It is contraindicated in severe or poorly controlled asthma. In patients with mild-to-moderate asthma or COPD where clinical benefit clearly outweighs risk, bisoprolol may be used cautiously with close monitoring for bronchospasm — but only under specialist guidance.
Drug Interactions
Bisoprolol may interact with other medications, including potassium-sparing diuretics and NSAIDs. Always inform your clinician about all medications you are taking.

Important Warnings
Abrupt withdrawal of bisoprolol in patients with ischaemic heart disease or heart failure is potentially life-threatening, causing rebound sympathetic activation, arrhythmias, and myocardial infarction. Always taper the dose gradually over at least 1–2 weeks under medical supervision.
Bisoprolol is contraindicated in severe asthma. Even in mild-to-moderate asthma, it must be used with extreme caution and only when the cardiovascular benefit clearly justifies the respiratory risk. Patients should be counselled to monitor for new or worsening respiratory symptoms.
Bisoprolol should not be initiated or continued if resting heart rate is below 50–55 bpm. Regular monitoring of resting heart rate is recommended, particularly during dose titration.
Bisoprolol can worsen symptoms of peripheral arterial disease and Raynaud's syndrome due to reduced peripheral blood flow. Use with caution in affected patients.
