What Is Apixaban (Eliquis)?
Apixaban is an orally administered direct oral anticoagulant (DOAC) — specifically a direct Factor Xa inhibitor — used for the prevention and treatment of thromboembolic conditions including stroke in atrial fibrillation, deep vein thrombosis (DVT), and pulmonary embolism (PE). It is manufactured by Bristol-Myers Squibb and Pfizer and marketed under the brand name Eliquis. Apixaban is available as 2.5 mg and 5 mg film-coated tablets.
Apixaban is the most widely prescribed DOAC in the UK for atrial fibrillation and shares this class with rivaroxaban, edoxaban, and dabigatran. Compared with rivaroxaban (once-daily for most AF indications), apixaban is taken twice daily. However, apixaban demonstrated favourable outcomes in the landmark ARISTOTLE trial — not only non-inferiority to warfarin for stroke prevention in AF but also superiority in reducing stroke rate, major bleeding, and all-cause mortality, making it one of the most evidence-supported anticoagulants available. Like all DOACs, it does not require routine INR monitoring and has fewer food interactions than warfarin.
Andexanet alfa (Ondexxya) is the specific reversal agent licensed for apixaban (and rivaroxaban) in the UK for life-threatening or uncontrolled bleeding.
What Conditions Does Apixaban Treat?
Apixaban is prescribed for:
Atrial fibrillation causes approximately 30% of all strokes. The irregular, disorganised atrial activity predisposes to clot formation in the left atrial appendage, which can embolise to the cerebral circulation causing large, disabling ischaemic strokes. Apixaban 5 mg twice daily (reduced to 2.5 mg twice daily in patients meeting dose reduction criteria — see below) significantly reduces the risk of stroke and systemic embolism in patients with non-valvular AF. The ARISTOTLE trial demonstrated that apixaban was superior to warfarin in reducing stroke and systemic embolism, with a significantly lower rate of major bleeding and intracranial haemorrhage.
The CHA₂DS₂-VASc score is used to stratify stroke risk in AF: anticoagulation is recommended for all men with a score ≥2 and all women with a score ≥3 (with consideration at lower scores on an individual basis). Apixaban is one of the DOACs recommended as first-line anticoagulation by NICE for non-valvular AF.
Apixaban is used for the acute treatment of DVT and the prevention of recurrent DVT and PE. The AMPLIFY trial demonstrated that apixaban was non-inferior to conventional therapy (LMWH followed by warfarin) for VTE treatment, with significantly less major bleeding. The treatment regimen uses a higher initial dose (10 mg twice daily for 7 days) to provide more intensive anticoagulation during the acute thrombotic phase, followed by a maintenance dose (5 mg twice daily for at least 3 months). For extended prevention of recurrence after the initial 6 months, a reduced dose of 2.5 mg twice daily can be used with demonstrated efficacy.
Apixaban treats acute PE using the same dosing regimen as DVT and avoids the need for initial parenteral heparin bridging required with warfarin — simplifying management and enabling earlier discharge in haemodynamically stable patients
While Apixaban is effective for preventing strokes and treating DVT, it should be used cautiously in patients with a history of bleeding disorders or hypersensitivity to the drug.
How Does Apixaban Work?
Factor Xa occupies a critical amplification point in the coagulation cascade — it converts prothrombin to thrombin (Factor IIa) within the prothrombinase complex, with each Factor Xa molecule capable of generating large amounts of thrombin. Thrombin then converts fibrinogen to fibrin, activates platelets, and amplifies further coagulation cascade signalling. Inhibiting Factor Xa therefore prevents clot formation at a highly efficient upstream point.
Apixaban directly and selectively inhibits both free Factor Xa (circulating in plasma) and Factor Xa bound within the prothrombinase complex. It does not require a cofactor (unlike heparin, which acts through antithrombin) and does not inhibit thrombin directly (unlike dabigatran). By blocking Factor Xa, apixaban prevents thrombin generation and the subsequent fibrin clot formation, while also reducing thrombin-mediated platelet activation.
Apixaban is absorbed within 3–4 hours of oral dosing, has a half-life of approximately 12 hours, and is eliminated by both renal (approximately 27%) and non-renal (hepatic and intestinal) routes — a dual elimination pathway that makes it less dependent on renal function than dabigatran and provides a degree of dosing flexibility in mild-to-moderate renal impairment.

Administration Tips
Apixaban tablets can be taken with or without food, at any time of day. For twice-daily dosing, consistent timing (e.g. with breakfast and with the evening meal) aids adherence. Tablets should be swallowed whole. For patients who cannot swallow tablets, apixaban tablets can be crushed and mixed with water or apple juice, or with apple purée, and administered immediately.
Side Effects of Apixaban
Apixaban is generally well tolerated, but some side effects may occur.

- Nausea
- Abdominal pain
- Major bleeding — a serious risk associated with all anticoagulants.
Patients should be informed about the risk of bleeding and the importance of not stopping Apixaban without medical advice.
Drug Interactions
Apixaban may interact with other medications, including strong dual CYP3A4 and P-gp inhibitors, increasing bleeding risk. Always inform your clinician about all medications you are taking.
Additive anticoagulant effects significantly increase bleeding risk. Concomitant use should generally be avoided unless specifically indicated under specialist supervision.
May significantly increase apixaban plasma concentrations, increasing the risk of serious bleeding. Avoid concurrent use or adjust therapy as clinically appropriate.
May reduce apixaban plasma concentrations and decrease anticoagulant effectiveness, increasing the risk of thromboembolism. Avoid concurrent use where possible.
Additive bleeding risk may occur when combined. Use cautiously with appropriate clinical monitoring.
Important Warnings
Seek urgent medical attention for any signs of serious bleeding: coughing or vomiting blood, blood in urine, black or tarry stools, severe unexpected bruising, prolonged bleeding from cuts, sudden severe headache, sudden vision changes, or weakness on one side of the body.
Stopping apixaban abruptly substantially increases the risk of stroke (in AF patients) or recurrent DVT/PE. If surgery or a procedure is planned, the prescribing clinician must be consulted for a structured interruption and restart plan.
Apixaban is partly renally excreted. Renal function should be assessed at least annually (more frequently in elderly patients or those with CKD), as declining renal function may alter drug levels and require dose adjustment.

Both are direct Factor Xa inhibitors with similar indications and broadly comparable efficacy and safety profiles. Key differences: apixaban is taken twice daily for AF (rivaroxaban once daily); apixaban demonstrated superiority over warfarin on the composite endpoint of stroke prevention, major bleeding, and mortality in ARISTOTLE; apixaban generally demonstrated lower GI bleeding rates in head-to-head comparisons. Both are appropriate first-line DOACs; clinical choice is guided by individual patient factors, prescriber familiarity, and compliance considerations.
The standard dose of 5 mg twice daily is appropriate for most AF patients. However, patients who are elderly (≥80), have low body weight (≤60 kg), or have elevated creatinine (≥133 micromol/L) have higher apixaban drug exposure and bleeding risk. In patients meeting at least two of these three criteria, the dose is reduced to 2.5 mg twice daily, which maintains stroke protection while reducing bleeding risk in these higher-risk groups.
Unlike warfarin, apixaban does not require routine INR monitoring. However, renal function and full blood count should be checked at least annually, as declining renal function increases apixaban drug levels and bleeding risk. Liver function should be assessed if symptoms develop. Blood tests should also be repeated after any significant change in medications, intercurrent illness, or significant change in body weight.
Moderate alcohol (1–2 units occasionally) does not significantly interact with apixaban pharmacokinetically. However, heavy or binge drinking significantly increases bleeding risk in patients on any anticoagulant and should be avoided. Alcohol can also impair medication adherence and compliance, which is important with twice-daily dosing.
Take the missed dose as soon as you remember on the same day. Do not take two doses in the same day to make up for the missed one. Resume the normal twice-daily schedule the following day. Because apixaban has a relatively short half-life of approximately 12 hours, consistent twice-daily dosing is important to maintain adequate anticoagulation throughout the 24-hour period.
